The finding of pupil-sparing third nerve palsy is synonymous with diabetic third nerve palsy in the minds of many clinicians. While this is the most common cause of a third nerve palsy with normal pupillary response, it is not the only cause. We present the case of an elderly diabetic gentleman who presented with pupil-sparing third nerve palsy and gait abnormalities without any weakness or incoordination in the extremities.

WEBINO (wall eye intranuclear ophthalmoplegia) is a specific type of neuroophthalmological condition that occurs due to a lesion in the MLF (medial longitudinal fasciculus), which causes unique symptoms characterized by bilateral adduction (inward movement of the eyes) impairment. Still, the abduction (outward movement of the eyes) may be preserved, and nystagmus during abduction is seen along with severe exotropia (outward movement of the eyes). In this case report, we report a 65-year-old male with a sudden onset of double vision, which he noticed while watching television, associated with a certain degree of vision loss and right-sided weakness of the body.

Brain cytotoxic edema is a neuroradiological sign secondary to variegate diseases ranging from migraine to fulminant Listeria rhombencephalitis. The tempestive identification of its underlying cause is vital for an effective treatment as any delay may be fatal. However, the lack of distinctive imaging biomarkers and the paucity of reports pose a significant challenge in its diagnosis and frequently lead to a misdiagnosis particularly with the more common acute ischemic stroke.

We retrospectively reviewed demographics, clinical and MRI data, of 163 patients, with MRI confirmed stroke. All patients underwent ECD TSA (Eco-Color-Doppler of the Supra-aortic Trunks) and TCCD (Transcranial Eco-color-Doppler), to study extra or intracranial stenosis, presence and composition of plaques.
