The duration of mechanical systole-also termed the flow time (FT) or left ventricular ejection time (LVET)-is measured by Doppler ultrasound and increasingly used as a stroke volume (SV) surrogate to guide patient care. Nevertheless, confusion exists as to the determinants of FT and a critical evaluation of this measure is needed. Using Doppler ultrasound of the left ventricular outflow tract velocity time integral (LVOT VTI) as well as strain and strain rate echocardiography as grounding principles, this brief commentary offers a model for the independent influences of FT. This framework establishes that systolic duration is directly proportional to the distance traversed by a single cardiac myocyte and indirectly proportional to its shortening velocity. Grossly, this translates to a direct relationship between FT and the LVOT VTI (i.e., SV) and an indirect relationship with mean ejection velocity. Thus, changes in the systolic time can infer SV change, so long as other cardiac parameters are considered.

Objective: Portosystemic shunts in growth-restricted fetuses are more common than previously thought. We aimed to describe fetuses with growth restriction and transient oligohydramnios in which a congenital intrahepatic portosystemic shunt (CIPSS) was noted during follow-up.
Methods: This was a retrospective study of all fetuses diagnosed with growth restriction and transient oligohydramnios during a 5-year period in a large tertiary referral center.

Transcranial Doppler is an instrumental ultrasound method capable of providing data on various brain pathologies, in particular, the study of cerebral hemodynamics in stroke, quickly, economically, and with repeatability of the data themselves. However, literature reviews from clinical studies and clinical trials reported that it is an operator-dependent method, and the data can be influenced by external factors, such as noise, which may require greater standardization of the parameters. Artificial intelligence can be utilized on transcranial Doppler to increase the accuracy and precision of the data collected while decreasing operator dependencies.

Introduction: Dynamic vertebral artery insufficiency is a rare vascular phenomenon characterized by insufficient flow of the posterior cerebral circulation induced by dynamic motion of the head. Prior case reports have been limited to vertebral artery occlusion by cervical osteophytes or other structural impingements.
Case Report: In this case report, we discuss the unusual case of a 61-year-old female with a history of vertigo, diplopia, nystagmus, and left subclavian stenosis admitted to the hospital for syncope elicited by a left head turn and left shoulder raise.
