Introduction: Persistent post-endodontic pain following endodontic therapy is a rare but substantial clinical challenge. Mechanisms that mediate peripheral neuropathy and/or central sensitization likely play a pivotal role in its development and chronification. The objective of this case series is to describe a management plan for persistent post-endodontic pain and provide prescription guidelines using a standardized "Gabapentin Pyramid Protocol" for clinical practice.

Methods: Four cases of persistent post-endodontic pain are presented. A peripheral etiology for pain was eliminated in all four cases. All patients were also refractory to conventional analgesics. Patients received the "Gabapentin Pyramid Protocol".

Results: Three of the four cases experienced complete resolution of symptoms within the prescribed treatment period. In one case, symptoms were greatly reduced but recurred upon discontinuation of the drug.

Conclusion: The "Gabapentin Pyramid Protocol" represents an effective treatment option for persistent post-endodontic pain, demonstrating significant symptom relief and potential for complete resolution.

Article Synopsis
  • The study aimed to determine how reducing occlusal surfaces affects postendodontic pain in patients with specific dental issues.
  • Sixty patients with acute pulpitis and apical periodontitis were randomly assigned to either reduce their occlusal surfaces or keep them unchanged, with pain levels measured over time using a visual analog scale.
  • Although pain levels decreased for both groups after treatment, there was no significant difference in pain levels between those who had surface reduction and those who did not.

Article Synopsis
  • The study aimed to evaluate how effective cold saline, curcumin, and regular saline are at reducing pain after endodontic treatment (root canal).
  • A total of 60 participants were divided into three groups to test each irrigant and pain levels were assessed at three different times using a visual analogue scale.
  • Results showed that both cold saline and curcumin significantly reduced post-treatment pain, demonstrating their effectiveness compared to regular saline.

To assess the overall impact of chlorhexidine, when used as an endodontic irrigant/medicament, on post-obturation pain following endodontic treatment by the application of Bayes methods of meta-analysis. This study aimed to combine recently documented evidence using rigorous and specific search criteria. The available data were analyzed using Bayesian methods, which have the advantage of accounting for uncertainties in estimates.
