Case report of preservative management of a giant broad ligament fibroid: A challenge in diagnosis and surgery.

Introduction: Broad ligament fibroids are unusual types of leiomyomas. They usually present as pelvic masses with pressure-related symptoms. The purpose of this case report is to shed light on the importance of keeping these fibroids in mind when approaching pelvic and abdominal masses, despite their low prevalence.

Case Presentation: Herein, we present a case of broad ligament fibroid in a 24-year-old female, who presented with gradual abdominal distension. On physical examination, an abdominal mass was palpated, extending from the hypogastric to the epigastric region of the abdomen. Ultrasound and Computed tomography (CT) were performed and described the size of the mass and its relation to the nearby organs. Surgery and histological examination were performed to arrive at the final diagnosis.

Discussion: Broad ligament fibroids can be asymptomatic or present with non-specific symptoms. Currently available imaging modalities can help determine the extent of the broad ligament fibroids but the definitive diagnosis is based on histological examination after the excision of the mass.

Conclusion: Broad ligament fibroids should be kept in mind as a differential diagnosis for pelvic and abdominal masses. Surgery of broad ligament masses should be performed while taking care not to cause injury to nearby organs.
