This paper is dedicated to Martin Kirschner one of the outstanding German surgeons of the first half of this century. Many of the inventions and improvements Kirschner made have stood the test of time till today. It was Kirschner who recommended to stretch the stomach for replacing the oesophagus. He practiced for the first time successfully an embolectomy out of the pulmonary artery (1924!). Kirschner improved the wire traction procedure decisevely. Of the same importance is his literary work.

Background: The use of online information and communication is globally increasing in the healthcare sector. In addition to known benefits in other medical fields, possible specific potentials of eHealth lie in the monitoring of oncological patients undergoing outpatient therapy. Specifically, the treatment with immune checkpoint inhibitors (ICI) requires intensive monitoring due to various possible negative side effects.

Inherited Telomere Biology Disorders: Pathophysiology, Clinical Presentation, Diagnostics, and Treatment.

Transfus Med Hemother

October 2024

Department of Hematology, Oncology, Hemostaseology and Stem Cell Transplantation, Medical Faculty, RWTH Aachen University, Aachen, Germany.

Article Synopsis
  • Telomeres protect chromosome ends from damage and shorten with cell division, which can lead to aging-related issues like cell senescence or death when they become critically short.
  • Premature aging and severe health complications, such as bone marrow failure and pulmonary fibrosis, can occur due to disorders affecting telomere maintenance, known as telomere biology disorders (TBDs).
  • Recognizing congenital causes of bone marrow failure in about 10% of cases is crucial for accurate diagnosis and treatment, emphasizing the need for proper counseling and donor selection for stem cell transplants.

Objectives: Despite major advances in treatment options for multiple myeloma (MM), patients refractory to the main drug classes and those with aggressive, especially extramedullary disease, still face a dismal outcome. For these patients, effective therapeutic options are urgently warranted.

Methods: In this retrospective study, we report on the safety and efficacy of the intensive combination regimen of pomalidomide plus cisplatin, doxorubicin, cyclophosphamide, and etoposide (Pom-PACE) in patients with relapsed refractory MM (RRMM) or plasma cell leukemia (PCL).

Evolutionary trajectories of small cell lung cancer under therapy.

Nature

March 2024

Department of Translational Genomics, Faculty of Medicine and University Hospital Cologne, University of Cologne, Cologne, Germany.

The evolutionary processes that underlie the marked sensitivity of small cell lung cancer (SCLC) to chemotherapy and rapid relapse are unknown. Here we determined tumour phylogenies at diagnosis and throughout chemotherapy and immunotherapy by multiregion sequencing of 160 tumours from 65 patients. Treatment-naive SCLC exhibited clonal homogeneity at distinct tumour sites, whereas first-line platinum-based chemotherapy led to a burst in genomic intratumour heterogeneity and spatial clonal diversity.

Telomere biology disorders may manifest as common variable immunodeficiency (CVID).

December 2023

Electronic address:

Telomere biology disorders (TBD) are caused by germline pathogenic variants in genes related to telomere maintenance and are characterized by critically short telomeres. In contrast to classical dyskeratosis congenita (DC), which is typically diagnosed in infancy, adult or late onset TBD frequently lack the typical DC triad and rather show variable organ manifestations and a cryptic disease course, thus complicating its diagnosis. Common variable immunodeficiency (CVID), on the other hand, is a primary antibody deficiency (PAD) syndrome.
