Bleomycin, a cytotoxic antibiotic commonly used as part of combination chemotherapy regimes in the treatment of germ cell tumors, is well known among clinicians for its potential pulmonary toxicity. Less well known, although postulated to occur through a similar pathomechanism, are the wide spectrum of dermatologic adverse reactions associated with bleomycin therapy. The current case report describes the sudden and distressing appearance of a pruritic erythematous flagellate "whip-like" rash in a 30-year-old Caucasian male undergoing treatment for testicular embryonal carcinoma following the second infusion of bleomycin-containing chemotherapy.

Flagellate dermatitis caused by bleomycin is a rare side effect with a distinctive pattern of whip-like, linear streaks. The clinical presentation has become uncommon nowadays as bleomycin use in conventional chemotherapy regimens has decreased. We present a case of a 30-year-old female diagnosed with ovarian germ cell tumour, managed with bleomycin, etoposide, and cisplatin (BEP) and later developed a widespread rash indicative of classic flagellate dermatitis.

Shiitake flagellate dermatitis is an intensely pruritic erythematous or violaceous eruption marked by "whip-like" linear streaks consisting of papules or plaques following the ingestion of undercooked shiitake mushrooms (Lentinula edodes). Most commonly localized on the trunk and upper limbs, the characteristic pattern of the rash is associated with the consumption of lentinan, a thermolabile toxin found within the mushroom, as well as a broad range of other dermatological conditions, including dermatomyositis, adult-onset Still's disease, and reactions to medications such as bleomycin. In this case report, we present a 24year-old active duty service member who presented to the clinic for the evaluation of a flagellate erythematous eruption on his abdomen, back, and extremities.

Bleomycin-induced flagellate erythema (FE), a skin finding associated with cutaneous deposition of bleomycin, is so called due to its characteristic pattern of whip-like, linear streaks. As bleomycin use in standard chemotherapeutic regimens has decreased, the clinical diagnosis has become increasingly rare. The authors present a case of a 43-year-old female patient with Hodgkin's lymphoma on her first cycle of adriamycin, bleomycin, vinblastine, and dacarbazine (ABVD) treatment, who subsequently developed a diffuse rash classic for FE.
