A 72-year-old man was diagnosed with posterior ventricular septal rupture after acute myocardial infarction with dissection in the posterior wall of the ventricular septum. On admission, cardiogenic shock was identified, and an Impella CP was inserted to improve his hemodynamics. Extended sandwich patch repair through the right ventricle with a Dacron patch was successfully performed, achieving an excellent postoperative course with stable hemodynamics. Preoperative Impella introduction may be a useful strategy for ventricular septal rupture repair in the acute phase.

Background: This study sought to determine the safety of primary and staged biventricular repair in neonates with interrupted aortic arch (IAA), ventricular septal defect (VSD), and severe left ventricular outflow tract obstruction (LVOTO).
Methods: Patients with a fundamental diagnosis of IAA and VSD between 2015 and 2020 were extracted from The Society of Thoracic Surgeons National Database by using a Participant User File. The objective was to compare outcomes for neonates undergoing primary and staged Yasui and Ross operations.

Among repairs for ventriculoarterial discordance, ventricular septal defect, and pulmonary stenosis, aortic root translocation (Nikaidoh operation) offers the most anatomic result. With a diminutive pulmonary annulus or hypoplastic left ventricular outflow tract, the distance gained posteriorly with aortic translocation is negligible. We developed the "hemi-Nikaidoh" procedure as an alternative.
