Serotonin toxicity is a potentially fatal condition caused by increased serotonergic activity in the central nervous system. Cyproheptadine, a serotonergic antagonist, is recommended for treatment; however, there is a lack of evidence to support its use. Berardinelli-Seip congenital lipodystrophy (BSCL), also known as congenital generalized lipodystrophy (CGL), is an exceptionally rare autosomal recessive disorder marked by a significant deficiency of adipose tissue throughout the body. This lack of adipose tissue, normally found beneath the skin and between internal organs, leads to impaired adipocyte formation and fat storage, causing lipids to accumulate in atypical tissues such as muscles and the liver. The extent of adipose tissue loss directly influences the severity of symptoms, which can include a muscular appearance, increased appetite, bone cysts, marrow fat depletion, acromegalic features, severe insulin resistance, skeletal muscle hypertrophy, hypertrophic cardiomyopathy, hepatic steatosis, hepatomegaly, cirrhosis, and intellectual disability.

Complete circumferential aortic dissection with bidirectional intimo-intimal intussusception is a rare occurrence in Stanford type A dissections. The antegrade dissection flap can obstruct the left ventricular outflow tract and coronary sinuses, whereas the retrograde flap can obstruct the aortic arch and branch vessels. Sequelae include aortic regurgitation, myocardial ischemia, and neurologic complications.

Necrotizing fasciitis is a deadly yet rare soft tissue and skin infection that is usually diagnosed clinically. At times, clinical signs may betray the underlying etiology and masquerade as cellulitis in the early course of the disease. We report four cases with clinical suspicion of necrotizing fasciitis, some after the failure of therapy for cellulitis who underwent 18-F fluorodeoxyglucose positron emission tomography-computed tomography (18-F FDG PET/CT) showing the extent of the disease, showing subclinical sites of involvement in patients with necrotizing fasciitis on baseline scan as well as its role in assessing response to treatment using 18-F FDG PET/CT.
