Alzheimers Dement

University of Southern California, Leonard Davis School of Gerontology, Los Angeles, CA, USA.

Published: December 2024

Background: The Alzheimer's Association (AA) is updating existing recommendations for the diagnosis of Alzheimer's disease (AD) to focus on biomarkers, independent of clinical syndrome. Although this perspective is consistent some other medical conditions (e.g., cancer) and has merit in research settings, there is concern about how the proposed approach will be implemented in clinical practice and how it might affect patients and families. Its on existing inequities in the diagnosis and treatment of AD are unclear.

Methods: Members of the Dementia Special Interest Group of the International Neuropsychological Society were surveyed about their opinions on these AA recommendations. The survey queried members about their demographics and asked them to rate their agreement with 15 statements related to the proposed recommendations (e.g., purpose of the recommendations, applicability to diverse groups in and outside of the United States, access to biomarker testing). The 15 statements were rated on a 4-point Likert scale (1 = Strongly Disagree to 4 = Strongly Agree), with six reverse-scored.

Results: Sixty-one respondents (67% female, 84% non-Hispanic, 79% white, 75% from United States) provided analyzable data. For 67% of statements, the full range of responses (i.e., 1 - 4) were observed. After recoding the statements, higher values reflected greater concern regarding the recommendations. The mean rating was 3.12 (SD = 0.36), suggesting that, overall, respondents had concerns about the recommendations. Statements that yielded the most concern focused on barriers to implementing the recommendations, including lack of healthcare resources and costs of biomarker testing (mean ratings: 3.52 - 3.61). Conversely, participants viewed updated diagnostic criteria as a positive step forward for the field and were equivocal about the applicability of criteria to diverse groups (mean ratings: 2.07 - 2.43).

Conclusions: In this survey of neuropsychologists and trainees who focus on dementia, there was concern about the proposed recommendations for diagnosing AD mainly on biomarkers. Limitations of this study included small sample size, ambiguous wording of some questions, and lack of a "neutral" response option. In conclusion, although reactions were varied, there is clear concern about how the AA's proposed approach to diagnosing AD would be implemented both inside and outside of the United States.

United States and European Union laws demand separate clinical studies in children as a condition for drugs' marketing approval. Justified by carefully framed pseudo-scientific wordings, more so the European Medicines Agency than the United States Food and Drug Administration, "Pediatric Drug Development" is probably the largest abuse in medical research in history. Preterm newborns are immature and vulnerable, but they grow.

Medical Termination of Pregnancy: Law and Controversies in the United States and Australia.

J Law Med

November 2024

Barrister, Castan Chambers, Melbourne, Australia; Professor of Law and Professorial Fellow in Psychiatry, University of Melbourne; Honorary Professor of Forensic Medicine, Monash; Adjunct Professor, Southern Cross University.

This editorial reviews the changes over two decades in the United States and Australia in relation to the law governing access to drugs enabling medical termination of pregnancy. It also scrutinises three contentious decisions by the United States Supreme Court between 2022 and 2024 in relation to abortion. It argues that the receptive environment in the United States Supreme Court, as it is currently constituted, to challenges to the lawfulness of terminations of pregnancy and abortion medications is likely to inspire comparable challenges as part of the "Abortion Wars" in other countries, including Australia.

Objective: Propose a framework to empirically evaluate and report validity of findings from observational studies using pre-specified objective diagnostics, increasing trust in real-world evidence (RWE).

Materials And Methods: The framework employs objective diagnostic measures to assess the appropriateness of study designs, analytic assumptions, and threats to validity in generating reliable evidence addressing causal questions. Diagnostic evaluations should be interpreted before the unblinding of study results or, alternatively, only unblind results from analyses that pass pre-specified thresholds.

Objective: We explored awareness of and attitudes about the safety of various methods people use to attempt to end a pregnancy without medical assistance, which we refer to in this study collectively as self-managed abortion (SMA).

Methods: In 2020, we invited individuals living in eight United States (US) states considered "hostile" to abortion rights or with a history of criminalizing abortions performed outside the formal healthcare system to participate in semi-structured telephone interviews regarding their attitudes toward these practices. We analyzed coded transcripts for content and themes.

TREM2 is a signaling receptor expressed on microglia that has emerged as an important drug target for Alzheimer's disease and other neurodegenerative diseases. While a number of TREM2 ligands have been identified, little is known regarding the structural details of how they engage. To better understand this, we created a protein library of 28 different TREM2 variants that could be used to map interactions with various ligands using biolayer interferometry.
