Osteochondral lesions of the talus are chondral lesions affecting the subchondral bone mostly due to acute ankle trauma, including either sprains or fractures. After failure of conservative treatment, operative treatment is necessary, with different surgical techniques described in the literature. We describe a single-step osteochondral autograft transfer to access the medial talar dome lesion that avoids the need for a medial malleolar osteotomy and therefore eliminates morbidity while reducing operative time.

Introduction: There are various surgical techniques applied for the treatment of osteochondral lesions of the talus (OLT). The mosaicplasty technique is one of the most commonly used methods for deep lesions. The all-arthroscopic technique with scaffold and autograft bone taken from the tibial plafond is a novel method as an alternative to the mosaicplasty.

Numerous surgical techniques have been developed to address recurrent anterior shoulder instability, with the Bankart repair and the Latarjet procedure emerging as dominant. With recent advancements in keyhole surgery, there's been a surge in popularity for all-arthroscopic procedures. Our systematic review aims to determine if there's justification for incorporating these techniques into a classification system for guiding treatment of traumatic anterior recurrent instability.
