A 12-year-old terrier was referred for investigation of a 4-month history of coughing, sneezing and nasal discharge. Clinical findings were consistent with sinonasal Aspergillus fumigatus infection with evidence of intracranial extension on computed tomography. Endoscopic debridement followed by topical clotrimazole and systemic antifungal therapy resulted in clinical improvement. Magnetic resonance imaging after 4 weeks showed reduced intracranial disease but demonstrated evidence of temporal myositis. Repeated debridement and topical treatment were performed at 4 and 8 weeks in conjunction with long-term voriconazole therapy. Further interventions were declined; the dog remains clinically well after 4 months with unilateral nasal discharge. As in human patients, invasive subtypes of sinonasal aspergillosis may also occur in dogs and be associated with poorer response to treatment.

Case Series Summary: Described are three cats diagnosed with rhinosinusitis secondary to , species and species, respectively. Medical records were retrospectively reviewed to identify cats with decreased nasal airflow and mucopurulent discharge that failed to improve on antibiotic therapy of 3 months or longer duration. Surgical debridement was followed by nasal packing using 5% povidone-iodine saturated umbilical tape, which was replaced at 24 h postoperatively.

Background: Dupilumab, an antibody that binds IL-4Rα and inhibits IL-4 and IL-13 signals, has demonstrated efficacy in chronic rhinosinusitis with nasal polyps (CRSwNP) primarily characterized by type 2 inflammation. Current evidence suggests that the rate of improvement in olfactory dysfunction with dupilumab exceeds that of nasal polyp reduction, yet the underlying mechanism remains undisclosed. We hypothesize that dupilumab may initially ameliorate sensorineural olfactory dysfunction.
