We report a case of symptomatic supraventricular tachycardia who had absent VA conduction during electrophysiology study. The interesting finding was appearance of VA conduction only at a specific cycle length of ventricular pacing which reproducibly induced a sustained orthodromic re-entrant tachycardia (ORT). We review the literature and conclude that supernormal AP conduction can explain such phenomenon.

A 51-year-old female with congenitally corrected transposition of great arteries (CCTGA), situs solitus, dextrocardia, atrial septal defect and persistent left superior vena cava underwent electrophysiology study for recurrent palpitations with documented narrow complex, short RP tachycardia. With a catheter in the region of the anterior mitral annulus, a His signal was recorded and HV interval was 35 msec. Tachycardia was induced with a ventricular extrastimulus.

Double-outlet left atrium is an extremely rare congenital ventriculo-atrial mal-alignment anomaly, wherein, the left atrium drains into both ventricles, through either a common atrioventricular valve or two separate atrioventricular valves. The only egress from the right atrium is through an inter-atrial communication. We present a 16-month-old male, diagnosed to have double outlet left atrium and describe its surgical management.
