AI Article Synopsis

  • Stroke is a major public health crisis in Iraq, with increasing cases but inadequate specialized care leading to high disability and mortality rates.
  • The editorial calls for the establishment of endovascular centers to provide effective treatments, like mechanical thrombectomy, which can significantly improve patient outcomes if administered promptly.
  • Iraq struggles with limited resources, infrastructure issues, and a shortage of trained professionals, but investing in endovascular centers could enhance stroke care and reduce the healthcare burden overall.

Article Abstract

Stroke is a critical public health issue in Iraq, where the rising incidence is met with insufficient specialized care, often leading to severe disability or mortality. This editorial highlights the urgent need for establishing endovascular centers across the country to address these gaps. Endovascular interventions, such as mechanical thrombectomy, have proven effective in reducing stroke-related mortality and long-term disability when administered within a narrow treatment window. However, Iraq faces numerous challenges, including limited resources, infrastructure deficits, and a shortage of trained specialists, compounded by socio-political factors that affect healthcare delivery. Iraq at this moment lacks a dedicated stroke endovascular center. By investing in endovascular centers and addressing the systemic challenges, Iraq can align its stroke care with global standards, offering improved outcomes for stroke patients and reducing the nation's overall healthcare burden.

Importance: Fenestrated and branched endovascular aortic repairs (F/BEVAR) have been adopted by many centers. However, national trends of F/BEVAR use remain unclear, particularly at sites who perform them without an US Food and Drug Adminstration (FDA)-approved investigational device exemption (IDE).

Objective: To quantify the use of F/BEVAR in the US and to determine if mortality was different at IDE vs non-IDE sites.

Clinical Outcomes of Endovascular Coil Embolization for Ruptured Middle Cerebral Artery Aneurysms.

J Neuroendovasc Ther

Department of Neurosurgery & Stroke, Institute of Clinical Medicine, University of Tsukuba, Tsukuba, Ibaraki, Japan.

Objective: Middle cerebral artery (MCA) aneurysms are difficult to treat with coil embolization (CE) due to their location and shape, but the number of CE-treated MCA has gradually increased as treatment techniques have improved. However, the outcomes of CE for ruptured MCA aneurysms are poorly understood. This study aimed to evaluate the outcomes of CE for ruptured MCA aneurysms.

Background: Clinically ineffective reperfusion (CIR) refers to the discrepancy between successful reperfusion and a favorable functional outcome in patients with large vessel occlusion (LVO) stroke after endovascular treatment (EVT). The Improving Neuroprotective Strategy for Ischemic Stroke with Sufficient Recanalization after Thrombectomy by Intra-arterial Cocktail Therapy (INSIST-CT) trial aimed to explore the safety, feasibility, and efficacy of intra-arterial cocktail therapy using argatroban, dexamethasone, and edaravone in patients who achieved sufficient reperfusion after EVT.

Methods: In this prospective, single-arm, pilot study, eligible patients with anterior circulation LVO who achieved sufficient reperfusion after EVT were enrolled in the INSIST-CT trial.

Background: Pulmonary embolism (PE) is an important cause of death and disability. Advances in catheter-directed therapies have led to the use of devices, such as the Inari FlowTriever and Penumbra Indigo system for aspiration thrombectomy (AT) for both massive and sub-massive PE. However, limited data exist on causes of procedural mortality.
