The ex vivo isolated heart perfusion model by Oskar Langendorff to resuscitate and maintain the automacity of a heart excised from a mammal was introduced in the late nineteenth century. It has remained an essential technique used for functional studies in cardiovascular research even after over a hundred years. This model provides information on contractility, cardiac rhythm, metabolism, and overall cardiac function. It allows the study of the heart in isolation without the influences of other organs or hormones. It enables the investigation of physiological mechanisms underlying heart function under different conditions with more experimental control than the heart in vivo. This chapter provides an overview of the protocol for the isolated heart perfusion model.

The volume of transcatheter aortic valve implantation (TAVI) for treatment of isolated aortic stenosis has far surpassed surgical aortic valve replacement (SAVR). There has been a consequent increase in TAVI explantation, now the fastest growing cardiac surgical procedure. Transcatheter heart valve explantation can be technically complex, with higher perioperative morbidity and mortality than routine SAVR or valve-in-valve TAVI.

Hyperhomocysteinemia (HHcy) is associated with the development and progression of chronic cardiovascular diseases through the deleterious effects of high levels of homocysteine (Hcy) on the cardiovascular system. However, the exact mechanism of action of Hcy on the acute injury of the cardiovascular system following ischemia/reperfusion (I/R) remains unclear. The present study demonstrated that copper mobilization occurs during cardiac I/R, and the interactive toxic effect of Hcy and mobile Cu during cardiac I/R induces necroptosis of cardiac microvascular endothelial cells (CMECs) and thus enhances cardiac dysfunction.
