This paper investigates unmet needs among dyads of people aged 50 and over in England. Understanding the extent and patterns of unmet needs for long-term care across different social groups is critical for understanding care-related experiences and inequalities and planning the long-term care system. Although spouses are a main source of care support, little is known about how spouses' characteristics relate to one's experience of unmet care needs. This study adopts a dyadic perspective, investigating the association between unmet care needs and spouses' characteristics, including socioeconomic status, health status and the quality of spousal relationships. Drawing on data from the English Longitudinal Study of Ageing (ELSA) (N = 3439), we matched the information of individuals who have care needs to the characteristics of their spouses and used random effects modelling to account for the longitudinal nature of the data. The results show that having a spouse with poorer functional abilities was associated with a higher risk of experiencing objective and subjective unmet needs. Men were more likely to experience objective unmet care needs if their spouses engaged in paid work, but this is not the case for women. Women faced a lower risk of subjective and objective unmet needs when having a closer relationship with their spouse, whereas this pattern was not observed among men. The findings highlight the importance of considering the interpersonal care relationships and gendered dynamics of caregiving, providing insights into designing gender-sensitive intervention programmes to better support people in care needs and their families.

Background: We have previously shown that there are 3 unique behavioral symptom clusters, or groupings of temporally related co-occurring behavioral and psychological symptoms (BPSD) representing how an individual's daily BPSD group together relative to their own typical BPSD manifestation across a 21-day period. To further validate these symptom cluster concepts, we examined whether they are predicted by different environmental triggers.
Method: Family caregivers completed daily diary surveys for 8 consecutive days reporting on the physical, social and care environment in addition to their care recipients different BPSD.

Background: Comprehensive Geriatric Assessment is the gold standard of clinical care for older patients but its application in the primary care setting is limited, possibly due to its time-consuming process. Hence, a brief geriatric assessment could be a feasible alternative. We conducted a scoping review to identify which brief geriatric assessment tools have been evaluated or implemented in primary and community care settings and to identify the domains assessed including their reported outcomes.

Data on the HIV care cascade demonstrated challenges in achieving Ending the HIV Epidemic (EHE) targets across all 18 EHE focus metropolitan areas, but innovative adherence interventions using point-of-care tenofovir testing and motivational interviewing support care cascade outcomes in Namibia and South Africa, respectively. Data on treatment with long-acting injectable (LAI) antiretroviral therapy (ART) demonstrated high acceptability, retention, and virologic suppression including in groups that were not well represented in clinical trials including persons born female and persons with detectable viral loads. The adjuvanted hepatitis B vaccine appeared to be safe and appeared to be superior to conventional hepatitis B vaccines in persons with HIV (PWH) who were prior nonresponders to the hepatitis B vaccine.

Study Objectives: This study examined the impact of central disorders of hypersomnolence (CDH) on family members of adult patients, the ways family members assist with managing CDH, and family members' utilization and satisfaction with information and support.
Methods: Participants were adults (N=100) with an adult family member diagnosed with idiopathic hypersomnia or narcolepsy. They completed a survey which included the Family Reported Outcome Measure (FROM-16), checklists, satisfaction ratings, and open-response questions.

Idiopathic pulmonary fibrosis (IPF) is often regarded as the archetypal progressive fibrosing interstitial lung disease (ILD). The term "progressive pulmonary fibrosis" (PPF) generally describes progressive lung fibrosis in an individual with an ILD other than IPF. Both IPF and PPF are associated with loss of lung function, worsening dyspnea and quality of life, and premature death.
