Since 2019, the Association of Clinical Research Organizations has conducted a landscape survey of risk based quality management (RBQM) adoption in clinical trials. Here, we present data from four years of surveys, with an emphasis on the most recent: the 2022 survey included data from 4958 trials across seven contract research organizations, of which 1004 were new studies started in 2022. Results indicate that while overall risk assessment adoption is strong, it is lagging in other risk-based components which suggests companies are not deriving the full expected benefits of performing a risk assessment and mitigation process to their trials. The 2022 study also suggests new study starts showing promising traction, with adoption hovering near 50% for most RBQM elements. At the same time, the survey suggests industry has mixed views on the potential value of quality tolerance limits (QTLs). Ultimately, centralized monitoring is being underutilized despite the potential of increased patient safety oversight and improved data quality. The authors of this paper developed a case study based on a trial in clinicaltrials.gov to demonstrate how RBQM adoption could include the key RBQM elements such as centralized monitoring, reduced source data review and source data verification as well as implementation of QTLs in a real-world scenario. The authors believe the clinical trial industry has an obligation to utilize centralized monitoring to produce more efficient and effective clinical trials and will make a case to do so in this paper.

Background: Community-based centers for people living with dementia (PLWD) have been implemented as part of the National Plan of dementia in Chile, delivering a multicomponent intervention to support PLWD and their families. There is limited knowledge on their implementation, and we had previously informed with respect to the implementation outcomes of the first center, called Kintun. This work offers an update on findings describing implementation outcomes of three more centers, with different implementation times and geographic locations in Chile.

We sought to examine the implementation and adaptation of these centers, transitioning from adult day services to a community-based dementia care model that has escalated to a public policy.
Method: We conducted a realistic evaluation using multiple qualitative data collection strategies and informants to analyze complex interventions, in 4 of a total of 9 centers.

There are no published guidelines regarding remotely training research assistants (RAs) to conduct neuropsychological tests. With technological advances allowing for increased international collaboration within the medical and research communities, challenges often arise from such partnerships, including linguistic, cultural, and physical barriers. A notable challenge for supervising neuropsychologists in international projects is the physical distance from RAs, sites, and materials, making training/supervision of RAs and monitoring test data quite challenging.

Liquid biopsy provides a minimally invasive approach to characterise the molecular and phenotypic characteristics of a patient's individual tumour by detecting evidence of cancerous change in readily available body fluids, usually the blood. When applied at multiple points during the disease journey, it can be used to monitor a patient's response to treatment and to personalise clinical management based on changes in disease burden and molecular findings. Traditional liquid biopsy approaches such as quantitative PCR, have tended to look at only a few biomarkers, and are aimed at early detection of disease or disease relapse using predefined markers.
