Article Synopsis
  • Intercoronary communication (ICC) is a rare coronary artery anomaly that occurs in only 0.002% of patients undergoing angiography, highlighting the need for clinical awareness.
  • A case study details a 56-year-old woman who experienced chest pain and was diagnosed with acute coronary syndrome; angiography revealed a significant stenosis and the existence of ICC, which connected two coronary arteries.
  • The study concludes that it's important to differentiate ICC from collaterals, as they have distinct anatomical characteristics and flow patterns, with collaterals usually forming due to obstructive disease.

Intercoronary communication (ICC) is a very rare coronary artery anomaly that connects directly 2 coronary arteries. This anastomosis is found between 2 nonobstructed coronary arteries with unidirectional or bidirectional blood flow. We report a case of a large ICC between the right coronary artery and the left circumflex artery in a patient with late stent thrombosis.

Coronary vasospasm is a well-known entity causing acute chest syndrome and can lead to myocardial infarction, ventricular arrhythmias, and even sudden cardiac death. While there are extensive case series showing the association of coronary vasospasm with cocaine, studies reporting marijuana-induced coronary vasospasm are limited in number. We herein present a case of coronary vasospasm in a middle-aged African-American male who presented to the emergency department after an episode of syncope.
