Substance use disorders are associated with structural and functional changes in the neuroendocrine, neuromediator, and neuromodulator systems in brain areas involved in the reward and stress response circuits. Chronic intoxication provokes emergence of somatic diseases and aggravates existing pathologies. Substance use disorders and somatic diseases often exacerbate the clinical courses of each other. Elucidation of biochemical pathways common for comorbidities may serve as a basis for the development of new effective pharmacotherapy agents, as well as drug repurposing. Here, we discussed molecular mechanisms underlying integration of visceral systems into the central mechanisms of drug dependence.

In 2012, a social issue arose concerning a high incidence of cholangiocarcinoma (bile duct cancer) among printing workers. The cause was prolonged exposure to high concentrations of 1,2-dichloropropane that was included in the ink cleaning agent. Until then, it was not known that this substance could cause cancer in humans.

Buprenorphine has superior safety in opioid use disorder compared with alternatives due to its action as a partial opioid agonist, which limits its ability to cause respiratory depression. There is a risk of precipitated opioid withdrawal after buprenorphine exposure in someone using full opioid agonists. Buprenorphine induction strategies that avoid precipitated withdrawal remain a crucial component for starting buprenorphine in individuals actively using opioids.

Objective: Although research supports an association between increased alternative reinforcement and decreased substance use, the impact of substance use on changes in reinforcement during posttreatment recovery remains untested. This study tested the effect of abstinence duration and substance use frequency on the trajectories of four reinforcement mechanisms, behavioral activation, reward probability, reward barriers, and valued living, from pre- to 12-month posttreatment.
Method: Adults in intensive outpatient substance use disorder treatment ( = 206) completed self-report measures of the four reinforcement constructs and substance use over six timepoints from pre- to 12-month posttreatment.
