Proportional modes to hasten weaning.

Purpose Of Review: The purpose of this review is to examine the current state of the evidence, including several recent systematic reviews and meta-analyses, to determine if proportional modes of ventilation have the potential to hasten weaning from mechanical ventilation for adult critically ill patients, compared to pressure support ventilation (PSV), the current standard of care during the recovery and weaning phases of mechanical ventilation.

Recent Findings: Proportional assist ventilation (PAV) and neurally adjusted ventilatory assist (NAVA) are two commercially available proportional modes that have been studied in randomized controlled trials (RCTs). Although several feasibility studies were not powered to detect differences in clinical outcomes, emerging evidence suggests that both PAV and NAVA may reduce duration of mechanical ventilation, intensive care unit (ICU) length of stay, and hospital mortality compared to PSV, as shown in some small, primarily single-centre studies. Recent meta-analyses suggest that PAV shortens duration of mechanical ventilation and improves weaning success rate, and NAVA may reduce ICU and hospital mortality.

Summary: The current state of the evidence suggests that proportional modes may hasten weaning from mechanical ventilation, but larger, multicentre RCTS are needed to confirm these preliminary findings.

We consider a problem of nonlinear response to an external electromagnetic radiation in conventional disordered superconductors which contain a small amount of weak magnetic impurities. We focus on the diffusive limit and use Usadel equation to analyze the excitation energy and dispersion relation of the collective modes. We determine the resonant frequency and dispersion of both amplitude (Schmidt-Higgs) and phase (Carlson-Goldman) modes for moderate strength of magnetic scattering.

In order to investigate the interfacial bonding properties of high-strength steel stainless wire mesh-reinforced ECC (HSSWM-ECC) and concrete, a finite element model was established for two types of interfaces based on experimental research. The results show that the failure modes observed in the 21 groups of simulations can be classified into three categories: debonding failure, ECC extrusion failure and concrete splitting failure. The failure mode was mainly affected by the type of interface.

This paper proposes a robust maximum power point tracking algorithm based on a super twisting sliding modes controller. The underlying idea is solving the classical trajectory tracking control problem where the maximum power point defines the reference path. This trajectory is determined through two approaches: a) using the simplest linear and multiple regression models that can be constructed from the solar irradiance and temperature, and b) considering optimum operating parameters derived from the photovoltaic system's characteristics.

Background: Applying convolutional neural networks to a large number of EEG signal samples is computationally expensive because the computational complexity is linearly proportional to the number of dimensions of the EEG signal. We propose a new Gated Recurrent Unit (GRU) network model based on reinforcement learning, which considers the implementation of attention mechanisms in Electroencephalogram (EEG) signal processing scenarios as a reinforcement learning problem.

Methods: The model can adaptively select target regions or position sequences from inputs and effectively extract information from EEG signals of different resolutions at multiple scales.
