Polymicrobial lower extremity necrotising fasciitis in a young toddler with sepsis.

BMJ Case Rep

Division of Pediatric Surgery, WVU, Morgantown, West Virginia, USA.

Published: November 2024

This is a case of a female toddler presenting with a minor injury to the right lower extremity with progressively worsening erythema and oedema. Initial workup revealed elevated inflammatory markers and imaging with significant soft tissue gas. The overall clinical presentation was highly suggestive of a necrotising soft tissue infection. The patient was expeditiously managed with fluid resuscitation, intravenous antibiotics and admission to intensive care. The patient required multiple operative interventions for excisional debridement of grossly necrotic soft tissue and fascia with eventual achievement of healthy wound margins amenable to reconstruction with skin grafting.

Introduction: Blunt brachial artery injuries (BAI) require reconstruction with an extensive vein graft due to the wide area of arterial damage. In the upper arm, safe options for pedicle flaps are limited, and selecting recipient vessels for free-flap surgery is challenging, complicating the treatment of soft tissue injuries associated with blunt BAI. This study aimed to analyze the characteristics and soft tissue reconstruction of blunt BAI and propose treatment strategies for treating associated soft tissue injuries.

The 5th Edition of the "WHO Classification of Tumours: Urinary and Male Genital Tumours" introduces several significant updates to the classification of testicular tumours. These updates include revised terminology for special germ cell tumour subtypes (neuroectodermal and neuroendocrine tumours) of the testis. Additionally, the signet-ring stromal tumour and myoid gonadal stromal tumour have been introduced as distinct entities within the sex-cord stromal tumours.

This retrospective cohort study examined the association between radiotherapy and prognosis in follicular lymphoma (FL) patients, with stratification by age, Ann Arbor stage, primary tumor site, surgical status, and grade. Using data from the SEER database, we employed Cox proportional hazards and competing risk models to assess the impact of radiotherapy on overall survival (OS) and cancer-specific survival (CSS) in 7,551 FL patients. The association between radiotherapy and second primary cancer (SPC) was explored using Logistic regression model.

Tibial plateau fracture is a fracture involving the proximal articular surface of the tibia, and its injury mechanism is complex, the fracture morphology is different, and it is often accompanied by different degrees of soft tissue injury, which is difficult to diagnose and treat. In recent years, the research hotspot has focused on solving the reduction and fixation of the posterior lateral column of the tibial plateau, because it has been clinically found that the residual sagittal plane after tibial plateau fracture is insufficient reduction or loss of reduction leads to knee joint dysfunction. The posterior inclination angle of the tibial plateau is an important parameter to describe the sagittal alignment of the tibia.
