Introduction: Tuberculosis (TB) remains a global health concern with diverse clinical manifestations. Parotid tuberculosis, although rare, presents diagnostic challenges due to its atypical presentation. Cytological examination is pivotal in diagnosing parotid lesions.

Case Presentation: We present three cases of parotid tuberculosis diagnosed through cytology. A 31-year-old male presented with a painless left parotid swelling. FNAC revealed granulomatous inflammation with AFB positivity. The second case, a 14-year-old female had an enlarging mass in the left parotid. FNAC showed granulomas, confirmed by GeneXpert MTB/RIF assay. The third case, a 39-year-old female presented with a painless swelling in the left parotid. FNAC demonstrated granulomatous inflammation, confirmed by mycobacterial culture.

Conclusion: Cytological examination plays a crucial role in diagnosing parotid tuberculosis. These cases underscore the importance of considering TB in parotid masses. Collaboration among clinicians, radiologists, and cytopathologists is essential for accurate diagnosis and prompt management. Further research is needed to optimize diagnostic algorithms for parotid tuberculosis.

Article Synopsis
  • Tuberculosis (TB) is a major global health issue, with extrapulmonary TB (EPTB) accounting for 15-20% of cases; diagnosing it is difficult due to nonspecific symptoms, especially in at-risk groups.
  • A case of a 25-year-old male with ear discharge and swelling led to imaging that revealed rare TB involvement in the parotid and mastoid regions, ultimately confirmed by microbiological testing showing Rifampicin-resistant mycobacteria.
  • The study highlights the need for high suspicion of TB in unusual infections, the importance of advanced imaging for diagnosis, and the necessity for microbiological confirmation to ensure effective and timely treatment.

Background: Extrapulmonary tuberculosis (TB) is a relatively rare form of tuberculosis infection, accounting for approximately 15% of all tuberculosis infections. Lymph nodes are the most commonly affected sites, while involvement of the parotid gland is extremely rare.

Case Presentation: We present the case of a 65-year-old male patient with a one-month history of a left parotid mass.

  • A Mantoux test confirmed tuberculosis, and a fine aspiration showed signs of inflammation and acid-fast bacilli.
  • After being started on a 4-drug antitubercular therapy, she showed significant improvement in pain and swelling within two months, highlighting the condition known as tubercular dactylitis which primarily affects younger children and presents as swelling of tubular bones.

Article Synopsis
  • Adenoid cystic carcinoma (ACC) is a type of aggressive salivary gland cancer primarily affecting the accessory, parotid, and submandibular glands, known for its slower growth and tendency for local recurrence.
  • A 71-year-old male patient from a remote area presented with discomfort and a lesion in his right submandibular gland after experiencing symptoms for four months.
  • After undergoing a biopsy that confirmed the presence of ACC, the tumor was successfully removed through a wide excision procedure.
