A 52-year-old man presented with a one-year history of episodic unilateral facial numbness, jaw weakness, ptosis, and hoarseness. Brain imaging was unremarkable and an 11.5cm right lower thoracic mass was discovered on chest x-ray, which was further evaluated by CT and MRI radiology. The lesion was biopsied, and histopathological examination revealed a thymoma. A diagnosis of ectopic thymoma with paraneoplastic myasthenia gravis was supported by positive anti-acetylcholine receptor and anti-titin serum antibodies. Paraneoplastic syndromes are rarely reported in association with ectopic thymoma, making this case unusual. We discuss the nuances of diagnosis, classification, and staging of this rare case, as well as the patient's subsequent management by cardiothoracic surgery and neurology specialists.

Intrathyroidal thymoma is a rare tumor that can be challenging to diagnose due to its unusual location and resemblance to more common thyroid conditions. We present the case of a 58-year-old woman with an incidentally discovered thyroid nodule during evaluation for an upper respiratory infection. Ultrasonography revealed an exophytic nodule in the left thyroid lobe, categorized as TR 3.

Article Synopsis
  • Myasthenia gravis (MG) is connected to thymus abnormalities, but the specific pathologic features in the thymus are not well understood.
  • This study utilized spatial transcriptome analysis to investigate thymoma (tumors of the thymus) and thymic hyperplasia (thymus enlargement) samples, revealing that most thymomas are primarily composed of cortex, while the medullary region is notably enlarged in seropositive cases.
  • The research highlights that the medulla contains unique immune structures and cell types that may be critical to MG pathology, suggesting these regions are important for future understanding and treatment of the disease.
