Supernumerary extensor tendons of the hand are relatively common, whereas supernumerary extensor pollicis longus (EPL) tendons are rarely reported. Herein, we present the case of a 17-year-old boy with painful snapping at the anatomical snuffbox after repetitive pushups. Preoperative dynamic ultrasonography and magnetic resonance imaging revealed the bifurcation of EPL tendons. Through explorative surgery under wide-awake anesthesia, we found 2 EPL tendons with a single EPL muscle, which pass through the single extensor compartment and EPL canal distally. Intraoperatively, after releasing the extensor compartment and EPL canal, each EPL tendon was rerouted to a separate gliding plane to resolve the snapping symptom and recurrence. Six months postoperatively, the snapping symptom was resolved, and the patient resumed activities of daily living without any complications. This clinical case may be useful for examining anatomical variations of the thumb extensor tendon and detecting extraordinary pathological symptoms.

Anatomical variations of the extensor pollicis longus (EPL) tendon are rare. Variations are typically asymptomatic, yet knowledge of these variations poses significance in the setting of dorsal approaches to wrist surgery. We present a case of an accessory EPL tendon that was discovered intraoperatively in the fourth dorsal compartment during open repair of a distal radius fracture with a dorsal spanning plate.

Supernumerary extensor pollicis longus (EPL) and extensor indicis proprius tendons are infrequently reported. There are few case reports on more than one accessory tendon in one individual. As a result, they are not represented in previously described classifications for variations in extensors to the thumb.

Therapy started in the sub-acute phase proved more effective, mainly when the patient participates actively. Recently, a novel set of rehabilitation and support robotic devices, known as supernumerary robotic limbs, have been introduced.

The presence of anatomical variants in the body may pose clinical challenges to inexperienced surgeons or clinicians and could cause misdiagnosis or treatment errors. Similarly, anatomical variations in cadavers pose educational dilemmas to students in gross anatomy dissection because of the inadequate coverage of anatomical variants in currently available resources, including textbooks. Students experience challenges in their learning experience, dissection process, and translation of clinically relevant information when presented with an anatomical variation.
