AI Article Synopsis

  • Syphilis is a sexually transmitted infection that can progress through multiple stages, with tertiary syphilis potentially affecting the cardiovascular system 10 to 40 years after the initial infection.
  • The case highlighted involves a 49-year-old man who experienced temporary vision loss, leading to hospital admission where imaging showed severe cardiovascular complications like aortitis and stroke.
  • A postmortem examination confirmed the diagnosis of tertiary syphilis, revealing syphilitic aortitis as the underlying cause of the patient's severe health issues.

Article Abstract

Syphilis is a sexually transmitted infection characterized by multiple stages. Cardiovascular involvement is a manifestation of tertiary syphilis, occurring between 10 and 40 years after the primary infection. The authors present a case of atypical presentation of tertiary syphilis in a 49-year-old male patient who was admitted to the hospital with bilateral transient loss of vision. Contrast-enhanced CT imaging revealed thoracic aortitis with carotid occlusion, coronary artery stenosis, ischemic stroke, myocardial infarction, and multiple intracardiac thrombi. A postmortem autopsy revealed positive laboratory results for syphilis, which was corroborated by medical autopsy findings of syphilitic aortitis. Neurosyphilis-induced dementia represents a severe manifestation of tertiary syphilis, characterized by cognitive and neuropsychiatric impairments. This condition arises from the progression of syphilis to the central nervous system, where the spirochete causes damage through invasion, chronic inflammation, and neurodegeneration. The pathophysiology involves chronic inflammatory responses, direct bacterial damage, and proteinopathies.

  • Venereal syphilis, caused by the bacteria Treponema pallidum, can escalate to serious complications like tertiary syphilis, which affects the heart decades after the initial infection.
  • A case study highlights a 46-year-old woman who developed life-threatening cardiac issues, including an ascending aortic aneurysm and severe aortic valve damage, due to untreated late-stage syphilis.
  • She underwent surgery to repair her aorta and replace the damaged valve, receiving antibiotics and intensive care; her recovery was satisfactory with no complications reported three months later.

Article Synopsis
  • The rate of syphilis cases has risen significantly over the past 20 years.
  • This case study discusses a 43-year-old man who presented with an aortic aneurysm and aortic regurgitation, initially thought to be a congenital issue.
  • Eventually, it was confirmed that the aortic problems were due to tertiary syphilis, highlighting the importance of considering syphilis as a potential cause of heart-related conditions.

The tertiary stage of syphilis appears to be the most diverse of the three recognized, with a number of cardiovascular, gummatous, neurological and psychiatric symptoms. This systematic review's aim is to analyze cases of organic psychoses secondary to tertiary syphilis, inspect the diagnostic procedures and assess the effectiveness of treatment. Case studies from Pubmed and MEDLINE Ultimate were sought out with the Boolean expression '((neurosyphilis) OR (syphilis) OR ()) AND (case report) AND ((psychosis) OR (psychotic))' to later undergo screening for exclusion criteria (according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) 2020 guidelines).
