Primary vaginal squamous cell carcinoma (SCC) is extremely rare. Primer vaginal SSC developed by the coeffect of leiomyoma and Human papillomavirus (HPV) is presented. We report a case of primary vaginal SCC in a 62-year-old woman presenting dyspareunia. On macroscopic examination, the surface of the operation material was partly ulcerated. On the cut surface, the material was solid, firm, white, and whorled. Microscopic examination revealed leiomyoma ulcerating the mucosa and SCC at the base of the ulcer. The case, in which vaginal SCC developed in the vagina due to the irritation of the leiomyoma, as well as its relationship with HPV, is important to be the first to our knowledge.
