Article Abstract

This case report highlights the prolonged SARS-CoV-2 reverse transcriptase polymerase chain reaction positivity in a 32-year-old immunocompromised male with a history of kidney transplants and chronic kidney disease. The whole genome sequencing of nasopharyngeal samples for SARS-CoV-2 collected 12 days apart showed the presence of the BA.1.1 Omicron variant. It revealed evidence of intra-host viral evolution, showing the development and loss of specific mutations over time. This report emphasizes the need for continuous monitoring strategies for immunocompromised patients, as they may serve as reservoirs for viral evolution and potentially give rise to immune escape variants.

This case report highlights the diagnostic utility of endocytoscopy (EC) in distinguishing high-grade squamous intraepithelial lesions (HSILs) from low-grade lesions (LSILs) in a 57-year-old male presenting with hematochezia. Traditional magnifying endoscopy was inconclusive; however, EC provided detailed visualization of cellular and vascular changes, facilitating a diagnosis of HPV-associated HSIL or carcinoma in situ.

Although steatotic liver onset after natural menopause has been reported, evidence on the clinical course and treatment options for steatotic liver after surgical menopause is scarce. A 34-year-old woman with a history of severe obesity presented to our department with liver dysfunction following total hysterectomy and bilateral oophorectomy. Her serum estradiol level was notably low at 22 pg/mL, and a liver biopsy revealed significant fatty degeneration, lobular inflammation, hepatocyte ballooning, and stage F1 fibrosis.
