NRG Oncology's Developmental Therapeutics and Radiation Therapy Subcommittee assembled an interdisciplinary group of investigators to address barriers to successful early phase clinical trials of novel combination therapies involving radiation. This Policy Review elucidates some of the many challenges associated with study design for early phase trials combining radiotherapy with novel systemic agents, which are distinct from drug-drug combination development and are often overlooked. We also advocate for potential solutions that could mitigate or eliminate some of these barriers, providing examples of specific clinical trial designs that could help facilitate efficient and effective evaluation of novel drug-radiotherapy combinations.

Purpose: As dual-phase amyloid-PET can evaluate amyloid (A) and neurodegeneration (N) with a single tracer injection, dual-phase tau-PET might be able to provide both tau (T) and N. Our study aims to assess the association of early-phase tau-PET scans and F-fluorodeoxyglucose (FDG) PET and their comparability in discriminating Alzheimer's disease (AD) patients and differentiating neurodegenerative patterns.
Methods: 58 subjects evaluated at the Geneva Memory Center underwent dual-phase F-Flortaucipir-PET with early-phase acquisition (eTAU) and F-FDG-PET within 1 year.

Background: For patients with small-size colorectal liver metastases, growing evidence suggests thermal ablation to be associated with fewer adverse events and faster recovery than resection while also challenging resection in terms of local control and overall survival. This study assessed the potential non-inferiority of thermal ablation compared with surgical resection in patients with small-size resectable colorectal liver metastases.
Methods: Adult patients (aged ≥18 years) from 14 centres in the Netherlands, Belgium, and Italy with ten or fewer small-size (≤3 cm) colorectal liver metastases, no extrahepatic metastases, and an Eastern Cooperative Oncology Group performance status of 0-2, were stratified per centre, and according to their disease burden, into low, intermediate, and high disease burden subgroups and randomly assigned 1:1 to receive either thermal ablation (experimental group) or surgical resection (control group) of all target colorectal liver metastases using the web-based module Castor electronic data capture with variable block sizes of 4, 6, and 8.

Aims: To assess the robustness of 4D-optimised IMPT and PAT plans against interplay effects in non-small cell lung cancer (NSCLC) patients with respiratory motion over 10 mm, and to provide insights into the use of proton-based stereotactic body radiotherapy (SBRT) for lung cancer with significant tumour movement.
Materials And Methods: Fourteen patients with early-stage NSCLC and tumour motion >10 mm were selected. Three hypofraction regimens were generated using 4D robust optimisation with the IMPT and PAT techniques.

Monitoring selenium (Se) concentrations in fish ovaries is an important tool for evaluating the ecological risk posed by Se in aquatic systems. Most guidance recommends sampling fish ovaries as closely as possible to when fish spawn on the premise that Se is mobilized from the liver to the ovary during vitellogenesis, and therefore, sampling ovaries during the early phases of oocyte maturation may underestimate egg Se concentrations at the time of spawning. In this study, we evaluated ovary Se data from two species with synchronous oocyte development (Ptychocheilus oregonensis and Prosopium williamsoni), one species with asynchronous oocyte development (Richardsonius balteatus), and one where the mode of development is unclear (Mylocheilus caurinus).
