Syphilis is often referred to as the 'great imitator' due to its diverse clinical manifestations throughout its clinical stages and polymorphic nature. We report a case of a 24-year-old man-who-has-sex-with-men presenting with an atypical syphilitic rash on the trunk, with a corymbiform appearance. This case highlights the necessity for clinicians to maintain a high index of suspicion for sexually transmitted infections, especially when there are skin manifestations of an uncertain nature.

"Corymbiform" is a term found in medical literature as early as 1876 to describe a central larger lesion with smaller surrounding lesions, leading to the appearance of an irregular border. While the term in current medical literature most often describes a possible morphology of secondary syphilis, the authors have noted this pattern presenting in other cutaneous conditions. We present a commentary on the corymbiform pattern in dermatology including a series of photographs of cutaneous disorders presenting in a corymbiform morphology in pediatric patients.

Syphilis, a sexually transmitted infection, may pose a challenge to diagnosis if it presents in an unusual form and in rare areas of the body. Non-typical lesions such as annular, maligna, nodular, nodular-ulcerative, corymbiform, leukoderma, pustular, berry-like, and chancriform presentations comprise about 29.6% of the skin manifestations of secondary syphilis.

A corymbose (or corymbiform) arrangement in secondary syphilis (derived from the Greek word ) is characterised by a central large plaque or papule surrounded by smaller satellite lesions akin to an explosion, whereas annular plaques consist of a peripheral ring of erythematous papules with central hyperpigmentation. These are very rare cutaneous manifestations of secondary syphilis. This case report focuses on one such interesting case which presented with the annular and corymbose pattern involving the face and genitalia.

It is essential that healthcare providers are familiar with the full spectrum of clinical presentations of syphilis. A rare manifestation of secondary syphilis is the corymbiform (or corymbose) arrangement, in which a central greater papule is surrounded by smaller satellite lesions. Very few reports of corymbiform syphilis are available in current biomedical databases.
