Background: Anastomotic stenosis after pull-through surgery remains a challenge in the management of Hirschsprung disease. Based on the management of esophageal stenosis, we evaluated the efficacy of combined radial incision and steroid injection for the treatment of refractory colorectal anastomotic stenosis after pull-through.
Impact Of Innovation: Combined radial incision and steroid injection is an alternative conservative treatment of refractory anastomotic stenosis after pull-though for Hirschsprung disease, avoiding a potential complicated redo pull-though surgery.
Technology Materials And Methods: We included patients with recto-sigmoid Hirschsprung disease that developed a refractory anastomotic stenosis after a laparoscopic-assisted Swenson pull-through at Robert-Debré Children University Hospital, Paris, France. Refractory stenosis was defined as obstructive symptoms associated with an anastomotic stenosis upon rectal exam without improvement after serial anal dilatations. Under general anesthesia, an injection of 10mg delayed-action steroid per quadrant was combined with a radial incision of the stenosis using monopolar cautery.
Preliminary Results: Combined radial incision and steroid injection was performed in 4 children, for either early or late refractory stenosis. This resulted in improvement of refractory anastomotic stenosis, avoiding a redo pull-through in 75% of patients. One child presented with transient improvement after combined radial incision and steroid injection but developed recurrent stenosis despite additional combined radial incision and steroid injection and redo pull-through. The median follow-up was 29 months.
Conclusion And Future Directions: We observed a clinical improvement in all the patients after combined radial incision and steroid injection. Steroids injection should be considered as a potential alternative therapy for anastomotic stenosis.

Electron transfer is ubiquitous in many chemical reactions and biological phenomena; however, the spatial heterogeneities of electron transfer kinetics in electrocatalysis are so far insufficiently resolved. Measuring and understanding the localized electron transfer are crucial to deciphering the intrinsic activity of electrocatalysts and to achieving further improvements in performance. By using scanning electrochemical probe microscopy to spatially resolve redox electrochemistry across the single-crystalline surface of gold microplates, we discover an intriguing radially distributed electron transfer pattern, where the kinetics around the periphery region are significantly higher than those at the central region, regardless of the redox reaction types.

Background: The presence of significant tortuosity in access routes to aneurysms can interfere with catheter guidance and manipulation and significantly impact treatment strategies.
Observations: In this report, the authors combined intentional staged aneurysm embolization with the construction of a new direct access route, which they call a "highway bypass," for a symptomatic posterior circulation cerebral aneurysm that was difficult to access with a catheter. Notably, the highway bypass is used for catheter passage, and technical tips should be considered.

We measure the response of open-cell polyurethane foams filled with a dense suspension of fumed silica particles in polyethylene glycol at compression speeds spanning several orders of magnitude. The gradual compressive stress increase of the composite material indicates the existence of shear rate gradients in the interstitial suspension caused by wide distributions in pore sizes in the disordered foam network. The energy dissipated during compression scales with an effective internal shear rate, allowing for the collapse of three data sets for different pore-size foams.

Genetic information is involved in the gradual emergence of cortical areas since the neural tube begins to form, shaping the heterogeneous functions of neural circuits in the human brain.
