Splenic ectopic pregnancy refers to the implantation of fertilized ovum in the spleen, which occurs extremely rare. Due to the abundance of splenic blood and its fragile texture, the lethality of ruptured hemorrhage in splenic ectopic pregnancy is higher than that of other ectopic pregnancies. Hence, prompt diagnosis and treatment are critical. Here, we report the case of a 34-year-old woman who was presented with amenorrhea. Computed tomography demonstrated a 1.4-cm solid-cystic lesion in the lower pole of the spleen with patchy and spotty enhancement. Therefore, a laparoscopic splenectomy was performed. Microscopically, the lesion contained placental villous tissues, diagnosing a splenic pregnancy.

Ectopic pancreatic or enteric tissue and invaginated splenic capsular epithelium are assumed to be the origin of mucinous cystadenomas of the spleen. We present the first reported case in Ethiopia.

Aim: Primary splenic pregnancy is a rare form of extratubal ectopic pregnancy, frequently associated with life-threatening abdominal bleeding, with only 51 cases reported in the worldwide literature. The aim of this work, through the proposal of its own index case, is to identify the fundamental steps of the diagnostic process, crucial to reduce the mortality rate, while evaluating the available therapeutic options, through a review of the literature, particularly the more conservative ones, to avoid splenectomy.
Methods: We present the case of a 22-year-old woman, showing her diagnostic-therapeutic pathway.
