The differential diagnoses of MAHA include thrombotic thrombocytopenic purpura (TTP), hemolytic-uremic syndrome (HUS), disseminated intravascular coagulation (DIC), idiopathic thrombocytopenic purpura (ITP), infections, malignancies, and solid organ transplantation. The commonly associated malignancies with MAHA are gastric, breast, prostate, lung, and lymphoma.

The underlying pathophysiology remains poorly understood, though hypotheses suggest abnormalities in the enteric nervous system or smooth muscle dysfunction. These conditions present significant diagnostic and therapeutic challenges, especially in cases refractory to conservative treatment.

A 66-year-old woman presents with unilateral orbital inflammation of several years of evolution, initially classified as idiopathic orbital inflammatory disease (IOID) subtype myositis. She presented with pain, eyelid inflammation and significant elevation of intraocular pressure, as well as radiological signs of exophthalmos and superior rectus myositis. She was treated with corticosteroids, with partial response.

We present а rare clinical case of successful surgical treatment - combined left-sided hemicolectomy and cecal resection for colon cancer of two locations in a patient with idiopathic thrombocytopenic purpura, which, in our opinion, is due to radiation exposure is presented. It is shown that the biological effects of long-term ionizing radiation on critical organs can have a diverse nature both in terms of the time of occurrence and in their structure, which requires an individual approach, the involvement of related specialists and the necessary medical preoperative preparation for precise planning of surgical intervention taking into account all possible risks.
