Chronic retracted rotator cuff tears are difficult entities to treat. L-shaped tears are a particular subset of such rotator cuff tears that pose challenges for surgeons attempting to reduce the supraspinatus tendon back to the greater tuberosity. Lack of full coverage of the tuberosity, need for medialization of the tendon, undue tension, and incomplete reconstitution of the rotator cable are some of the reasons L-shaped retracted tears of the supraspinatus can be challenging.

The ideal treatment approach to massive, irreparable rotator cuff tears remains a hot topic of debate. In addition to joint-preserving methods including partial repair and subacromial spacer, techniques such as superior capsular reconstruction and tendon transfers are viable alternatives. These techniques are effective in providing pain relief and-to an extent-functionality during short-term follow-up; however, superior capsular reconstruction (SCR) appears to be the treatment of choice to address the pseudoparalysis.
View Article and Find Full Text PDFThe goal of shoulder superior capsular reconstruction and/or anterior cable reconstructions, at least in terms of biomechanics, is to primarily restore a fulcrum to assist with pain control and functional optimization, with the secondary hope of maintaining cartilage. Fully restoring glenohumeral joint loads with SCR cannot be expected in the setting of persistent tendon insufficiency. Biomechanical studies characterizing shoulder capsular reconstructions have demonstrated anatomic and functional restorations toward normalization when tested with standard biomechanical methods.
