Background: End-stage kidney disease (ESKD) patients undergoing hemodialysis experience diverse neurological complications. This study investigated prefrontal cerebral blood volume (CBV) and cerebral blood flow (CBF) during hemodialysis using functional near-infrared spectroscopy (fNIRS) to analyze cerebral hemodynamic changes.
Methods: ESKD patients undergoing maintenance hemodialysis without a history of neurological disorders were enrolled prospectively. The fNIRS data were collected using a NIRSIT Lite device. The fNIRS values were recorded three times for each patient: before the start of hemodialysis (pre-HD), 1 h after the start of hemodialysis (mid-HD), and after the end of hemodialysis (post-HD). The average changes in oxy-hemoglobin (HbO), deoxy-hemoglobin (HbR), total hemoglobin (HbT, calculated as HbO + HbR) concentrations, and in hemoglobin concentration difference (HbD, calculated as HbO - HbR) were analyzed. We then compared the differences in changes in HbO, HbR, HbT, and HbD according to the hemodialysis period.
Results: Thirty hemodialysis patients were analyzed. The change in HbO, HbT, and HbD levels showed significant differences according to the hemodialysis period. Between the pre-HD and post-HD periods, there were significant differences in changes in HbO (0.005 ± 0.001 µM vs. 0.015 ± 0.004 µM, = .046) and HbT (0.006 ± 0.001 µM vs. 0.016 ± 0.008 µM, = .029). Additionally, between pre-HD and post-HD periods, HbD tended to increase (0.005 ± 0.001 µM vs. 0.014 ± 0.004 µM, = .094).
Conclusions: We demonstrated that during one hemodialysis session, the relative change in prefrontal CBV increased post-HD compared with pre-HD. These results are expected to help understanding the mechanisms underlying the effects of hemodialysis on brain function.

Extracorporeal membrane oxygenation (ECMO) is a technique used to support severe cardiopulmonary failure. Its potential life-saving benefits are tempered by the significant risk for acute brain injury (ABI), from both primary pathophysiologic factors and ECMO-related complications through central nervous system cellular injury, blood-brain barrier dysfunction (BBB), systemic inflammation and neuroinflammation, and coagulopathy. Plasma biomarkers are an emerging tool used to stratify risk for and diagnose ABI, and prognosticate neurofunctional outcomes.

Population exposure to plastics is increasing, and plasticizers are frequently detected in humans as important ingredients of plastic products. However, patterns of exposure to harmful ingredients of plastics and their effects on neurofilament light chain (NFL), a marker of active brain pathology, are currently inconclusive. Herein, we employed a range of statistical methods to thoroughly investigate the impact of 24 plastic hazardous ingredients and their varying exposure patterns on NFL concentrations in the blood of the general population in 533 participants.

Background: In China, stroke is the primary cause of adult death and disability. Because of the increased rate of blood vessel reperfusion, it is important to prevent cerebral ischemia-reperfusion injury, in which glutamate (Glu) excitotoxicity plays a critical role. The most important Glu transporter, GLT-1, is essential for the regulation of Glu, which is dependent on Na-K-ATPase (NKA)-induced ion concentration gradient differences.

Despite advances in procedural techniques and equipment, postdural puncture headache (PDPH) remains a serious complication of labour epidural analgesia after accidental dural puncture (ADP). Often considered a temporary inconvenience, PDPH can be debilitating in the short term. It can also be associated with chronic manifestations and serious complications.

Among contributors to diffusible signaling are portal systems which join two capillary beds through connecting veins. Portal systems allow diffusible signals to be transported in high concentrations directly from one capillary bed to the other without dilution in the systemic circulation. Two portal systems have been identified in the brain.
