Advanced endoscopy has been shown to be useful in the diagnosis and treatment of both benign and low-grade malignant colorectal lesions. In fact, advanced endoscopic procedures are being adopted as standard approaches to these lesions in many places around the world; however, their implementation in the United States has not been as widespread. We ascribe the difficulty in implementation to two reasons: (1) lack of advanced endoscopic training and (2) failure in reimbursement models as they relate to endoscopy. In this article, we hope to describe these barriers and inspire colorectal surgeons to try and overcome them. As surgical specialists with a mastery of endoscopic techniques, colorectal surgeons would be able to maximize benefit for their patients and minimize health care costs in the long run.

Introduction: Colonoscopies are routine procedures performed primarily on adults over the age of 50; however, there is little known about the influence of social determinants of health on successful completion of colonoscopies. Inadequate at-home bowel preparation can result in increased procedure duration, decreased cancer detection, and may necessitate a repeated colonoscopy, putting undue stress on the patient. Research suggests neurocognitive disorder is a risk factor for poor bowel preparation in older adults; however, lower education may confound neurocognitive findings, independently contributing to risk of incomplete colonoscopies.

The rising prevalence of obesity has led to a substantial investment in the advancement of treatment options for the disease and its comorbid conditions including lifestyle, pharmacologic, and procedural interventions. In this study, we describe a patient with a history of Roux-en-Y gastric bypass who was diagnosed with ischemic jejunitis on upper endoscopy because of the development of an internal hernia, a known late complication of bariatric surgery. This case highlights the diagnostic utility of endoscopy in postgastric bypass complications and the need for safer alternatives to surgery that have the potential to achieve significant weight loss such as bariatric endoscopic therapies.

Piriformis syndrome (PS) is an underdiagnosed condition, caused by entrapment of the sciatic nerve by the piriformis muscle tendon and adhesions in the deep gluteal space. We present a step-by-step endoscopic technique with the patient in a prone position through a posterior approach. This approach provides improved orientation for tracking the sciatic nerve from distal to proximal, facilitating the release of all adhesions and concluding with a piriformis tendon release.
