Despite a lower estimated rate of cancer incidence in Iran compared to the global average, the trend is unfortunately increasing. This necessitates the implementation of early detection of cancer and targeted therapies to effectively treat various types of cancer. Therefore, the 5th "International Royan Cancer Conference: From Bench to Bedside" was held to focus on critical cancer-related aspects such as gene- and cell therapy, immunotherapy, oligonucleotides in cancer treatment, biosensors for detection, and drug delivery. The 2-day conference took place in February 2024 at the Royan Institute, Tehran. This collaborative effort brought together experts from both basic and clinical research fields. The primary objective of the conference was to address clinical challenges and harness the potential of basic sciences for early cancer diagnosis and treatment, with a robust emphasis on ethical considerations. The conference aimed to ensure optimal patient care while advancing scientific understanding in the field and facilitating effective research collaborations among researchers and enthusiasts dedicated to combating cancer.

Personalized medicine is an emerging field that provides novel approaches to disease's early diagnosis, prevention, treatment, and prognosis based on the patient's criteria in gene expression, environmental factors, lifestyle, and diet. To date, hepatocellular carcinoma (HCC) is a significant global health burden, with an increasing incidence and significant death rates, despite advancements in surveillance, diagnosis, and therapeutic approaches. The majority of HCC lesions develop in patients with liver cirrhosis, carrying the risks of mortality associated with both the tumor burden and the cirrhosis.

Extracellular vesicles (EV), including exosomes or small EV (sEV) derived from biological fluids, such as milk, have garnered increasing interest in veterinary medicine because of their role in the pathophysiology and understanding of the disease status of the host. Bovine milk serves as a rich source of sEV, containing diverse cargoes of nucleic acids, proteins, and lipids, which play a critical role in intercellular communication and regulation of host status. Although it is more difficult to isolate and purify sEV from bovine milk than from human breast milk, challenges persist in enabling the enrichment and analysis of sEV populations, facilitating the elucidation of their functional roles and prognostic potential in cattle diseases.
