Objective: Pulmonary artery sarcoma (PAS) is an exceedingly rare and insufficiently investigated disease, leading to uncertain in its optimal management. This study aims to present our institutional experience and the outcomes of pulmonary endarterectomy for PAS.

Methods: We gathered clinical characteristics, intraoperative data, postoperative outcomes, and prognosis information from PAS patients who underwent surgical treatment at our institution between December 2016 and September 2023.

Results: A total of 20 patients with PAS underwent pulmonary endarterectomy. The median age of the patients was 52 (IQR 45, 57) years, with 12 patients (60%) being female. Intimal sarcoma was confirmed in 19 patients, while the remaining one was diagnosed with large cell neuroendocrine carcinoma. The perioperative mortality rate was three cases (15%). Follow-up was conducted for a median duration of 14 months (range: 1-61). During the follow-up period, 11 patients experienced recurrence or metastasis, and 5 patients succumbed to the disease. The estimated cumulative survival rates at 1 and 2 years for all 20 patients were 66.4% and 55.3%, respectively.

Conclusion: Pulmonary endarterectomy emerges as a palliative but effective approach for managing PAS, particularly when complemented with postoperative therapies such as chemotherapy and targeted therapy, which collectively contribute to achieving favorable long-term survival outcomes.

Objective: The purpose of this study is to identify variables at the time of clinical presentation which place patients at higher risk for mortality following carotid endarterectomy (CEA) for symptomatic lesions. Further, this study will create a risk score for mortality within two years following CEA for symptomatic stenosis to help tailor future postoperative and long-term management by identifying patients who require heightened vigilance in postoperative care to facilitate survival.

Methods: The Vascular Quality Initiative (VQI) CEA module was queried for procedures performed for symptomatic (within 180 days) carotid bifurcation stenosis.

Background: We retrospectively analyzed the patients who underwent prone positioning (PP) for acute respiratory failure after pulmonary endarterectomy (PEA).

Methods: One hundred twenty five patients underwent PEA and the outcome related to patients who underwent PP for acute respiratory failure after surgery was analyzed.

Results: Thirteen patients (10%) underwent PP at the mean duration of 28.

Objective: We aimed to explore the feasibility of an inverted-T upper hemisternotomy approach for pulmonary endarterectomy (PEA) and report the results after 17 cases.

Methods: PEA was conducted through a 7-cm skin incision using an inverted-T upper hemisternotomy across the third intercostal spaces. Cardiopulmonary bypass (CPB) was established through central arterial and percutaneous femoral dual-staged venous cannulation.

Background: Allergy to iodine contrast agents has long been a contraindication for balloon pulmonary angioplasty (BPA). We report the successful zero-contrast BPA procedure of a patient with inoperable chronic thromboembolic pulmonary hypertension (CTEPH) and severe iodine allergy using real-time fusion of computed tomography angiography (CTA) with x-ray fluoroscopy.

Case Presentation: A 62-year-old woman with CTEPH who developed a severe allergic reaction after the first BPA procedure.

Pulmonary artery sarcoma is a rare intravascular tumor that mimics pulmonary embolus. Early recognition and referral to surgery is important because nonsurgically treated tumors have a poor prognosis. Here, we describe a case of pulmonary artery sarcoma diagnosed with multimodal imaging, which also aided in surgical planning.
