Background: Emergency pancreaticoduodenectomy (EPD) is a rare event for complex periampullary etiology. Increased intraoperative blood loss is correlated with poor postoperative outcomes.
Case Summary: Two patients underwent EPD using a no-touch isolation technique, in which all arteries supplying the pancreatic head region were ligated and divided before manipulation of the pancreatic head and duodenum. The operative times were 220 and 239 min, and the blood loss was 70 and 270 g, respectively. The patients were discharged on the 14 and 10 postoperative day, respectively. Thirty-two patients underwent EPD for the treatment of neoplastic bleeding. The mean operative time was 361.6 min, and the mean blood loss was 747.3 g. The complication rate was 37.5%. The in-hospital mortality rate was 9.38%.
Conclusion: The no-touch isolation technique is feasible, safe, and effective for reducing intraoperative blood loss in EPD.

This article highlights the critical importance of identifying the classic triad of hemoptysis, anemia, and diffuse pulmonary infiltrates, offering clinicians a structured approach for the timely diagnosis and management of the diffuse alveolar hemorrhage in setting of GPA. Post-intubation HRCT findings revealed diffuse patchy ground glass opacities in both lungs, along with right lobar consolidation showing liquefaction and an air-fluid level.

Short stature, joint hyperextension, ocular hypotension, Rieger abnormalities, and delayed tooth eruption (SHORT) syndrom is a rare primary autosomal dominant genetic disorder mainly caused by pathogenic loss-of-function variants in the phosphoinositide-3-kinase regulatory subunit 1 (PIK3R1) gene. We report the case of a Chinese adult female patient with SHORT syndrome, carrying a PIK3R1 gene variant (c.1945C > T), who developed abnormal glucose metabolism and severe postprandial insulin resistance over 9 years.

Inappropriate birth weight for gestational age (IBWGA) is linked with obstetric complications like birth asphyxia, hypothermia, and postpartum hemorrhage. This study was aimed at determining the prevalence of IBWGA with factors associated with newborns born at Dessie Referral Hospital, northeast of Ethiopia. We used a retrospective cohort study design and systematic random sampling method to select charts of women giving birth at the hospital from January 2013 to December 2017.

Microglial-mediated neuroinflammation is crucial in the pathophysiological mechanisms of secondary brain injury (SBI) following intracerebral hemorrhage (ICH). Mitochondria are central regulators of inflammation, influencing key pathways such as alternative splicing, and play a critical role in cell differentiation and function. Mitochondrial ATP synthase coupling factor 6 (ATP5J) participates in various pathological processes, such as cell proliferation, migration, and inflammation.
