Background: Ceramic-on-ceramic (CoC) bearings have been increasingly used in total hip arthroplasty (THA) because of their superior wear resistance and biocompatibility. However, there is a scarcity of reports on the computed tomography (CT) evaluation of CoC bearings with more than 10 years. The aim of this study was to evaluate the long-term CT results of THA using CoC bearings for more than 20 years of follow-up. We hypothesized that there would be no wear, osteolysis, or ceramic fracture.

Methods: Between November 1997 and June 2003, 956 hips underwent THA using alumina-on-alumina bearings at a tertiary referral hospital. Among them, 107 hips were assessed, all of which underwent a CT examination more than 20 years after the index surgery. The mean age at the time of surgery was 41 years, and a CT scan was performed at an average of 22.0 years postoperatively (range, 20.0 to 25.1). The CT scans were thoroughly assessed for osteolysis, stem notching, and ceramic component fracture.

Results: No loosening was observed in the acetabular cup or femoral stem. Stem notching was observed in 3 hips (2.8%). In the CT scan taken after a minimum of 20 years of follow-up, 1 case (0.9%) of osteolysis around the cup and 2 cases (1.9%) of osteolysis around the femoral stem were noted. Suspected chip fractures of the ceramic insert were discovered in 4 cases (3.7%). Despite these findings, the patients remained asymptomatic, and no subsequent surgical intervention was needed after close follow-up.

Conclusions: Routine CT examinations for patients who underwent THA using CoC bearings over 20 years ago revealed unexpected findings, such as osteolysis and suspected chip fractures of the ceramic liner. However, routine CT scans may not be universally necessary. The CT evaluation in this cohort should be selectively performed for patients who have relevant clinical symptoms.

Level Of Evidence: Level III, Therapeutic study.

Large-diameter heads (LDHs) in total hip arthroplasty (THA) enhance range of motion but require thinner liners. Monoblock acetabular components with ceramic liners could reduce liner fracture risks during modular acetabular component assembly. This study aims to confirm the safety and clinical performance of the monoblock Maxera Cup in THA.

Some biomasses like cotton contain natural fibrous structures. This is a desirable structural feature for exposure of adsorption sites on cotton-derived activated carbon (AC). This was verified herein by conducting activation of cotton with ZnCl, HPO, KCO or KOH, probing whether structural transformation during activation could be confined inside a cotton fiber.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disease characterized by recurrent painful nodules, abscesses and sinus tract on the apocrine gland-bearing skin. The role of sex hormones in the pathogenesis as well as the use of hormonal treatment in the management of HS is still debated. We performed a retrospective cohort study including 183 patients to assess the influence of combined estrogen-progestin oral contraceptive (COC) in HS onset and disease severity.

Article Synopsis
  • - The study focuses on how Co(III) acts as a catalyst in reactions between N-chlorobenzamide and various olefins, analyzing how different electron properties of these olefins affect the reaction outcomes.
  • - A key finding is that the step where the alkene inserts into the reaction dictates the overall regioselectivity, meaning it influences which products are formed.
  • - The energy decomposition analysis (EDA) reveals that when using styrene, a specific insertion is favored due to less repulsion between the particles involved, while a different insertion pathway is preferred with vinyltrimethylsilane because of its higher electron density enhancing stabilizing interactions.

Herein, we report a novel strategy of hypervalent iodine(III) compound-mediated selective C-C bond cleavage of alkynes and C═N/N-O bond cleavage of nitrones and recombination of C-C/C-O/C-N multiple bonds to access various functionalized [1,4]oxazinones bearing a vicinal carbon stereocenter in good yields and high diastereoselectivity. Mechanistic studies revealed that the reaction undergoes a domino [4 + 3] cycloaddition, 1,3-rearrangement of N-O bond, intramolecular cyclization, dearomatization, and rearomatization over four steps in a single flask. The present method features good functional group tolerance, broad substrate scope, and C-C/C═N/N-O multiple bonds cleavage and recombination.
