•We herein present a case of chronic progressive autoimmune GFAP astrocytopathy.•Symmetrical high-intensity signals on FLAIR were observed in the white matter of the temporal and occipital lobes, lateral cerebral ventricle walls, hippocampus, amygdala, and occipital cortex, with extensive Gd enhancement in radial perivascular lesions and the ependyma in the choroid plexus.•Improvements were achieved by 4 courses of IVMP and one of IVIg.

Juvenile dermatomyositis with emphasized vasculopathy is rare, but the most severe form of the disease, with a poor prognosis with relapsing and chronic course or, in some cases, lethal outcome. We present a case of a 19-year-old Caucasian female, who developed severe acute juvenile dermatomyositis with emphasized multisystem vasculopathy, including retinal vasculopathy and maculopathy (cotton-wool spots, retinal hemorrhages, macular edema) at the age of 8. Due to no response to standard treatment protocols and rapid worsening of clinical symptoms and laboratory findings, a TNF inhibitor (infliximab) was introduced after the third week of treatment resulting in complete normalisation of muscle enzyme levels and complete resolution of eye changes within the next 2 weeks with a gradual general recovery.

Papulonodular mucinosis is a rare dermatological condition characterized by mucin deposition in the dermis, leading to the formation of papules and nodules that can occur with, or antedate, autoimmune connective tissue diseases. This case report presents a 67-year-old female with a chronic history of cutaneous mucinosis, which posed significant diagnostic challenges. Despite various treatments and extensive diagnostic workup, her condition evolved, highlighting the difficulties in diagnosing papulonodular mucinosis, especially in the absence of systemic lupus erythematosus and antinuclear antibody positivity.

Hidradenitis suppurativa mainly affects patients at a young age and is very often refractory to conventional medical treatment.
