Pelvic tilt (PT) is an important parameter for orthopedic surgeries involving hip and spine, typically determined from sagittal pelvic radiographs. However, various challenges can compromise the feasibility of measurement from sagittal imaging, including obscured landmarks, anatomical variations, hardware interference, and limited medical resources. Addressing these challenges and with the aim of reducing radiation exposure to patients, our study developed a novel method to estimate PT from antero-posterior (AP) radiographs, using vertical distances from the pelvic outlet and obturator foramen. We correlated these measurements with PT, defined both anatomically (anterior pelvic plane, PT) and mechanically (centers of femoral heads and sacral plate, PT). The study explored creating linear, exponential, and multivariate regression models based on twelve 3D CT-derived pelvic models (six men, six women), simulating AP radiograph projections with controlled PTs. We then validated these models against 105 pairs of patient stereoradiographs. Statistical analysis revealed that combined exponential-linear models yielded the most accurate results, with Pearson correlation coefficients of 0.75 for PT and 0.77 for PT, and mean absolute errors of 3.7° ± 2.6° for PT and 4.5° ± 3.4° for PT, showing excellent measurement reliability (all ICCs > 0.9) without significant gender discrepancies. In conclusion, this study presents a validated, simple, and accessible method for estimating PT using AP radiograph parameters, supported by the Supporting Information S1: Excel Tool, showing great potential for clinical application in hip and spine procedures.

Objective: Determining the optimal osteotomy length for patients with unilateral Crowe-IV developmental dysplasia of the hip undergoing subtrochanteric osteotomy remains challenging due to the significant variability in pelvic and spinal alignment. Incorrect osteotomy length, compounded by pelvic or spinal tilt, can adversely affect postoperative gait and long-term outcomes. Therefore, this study could introduce a method to calculate the osteotomy length for patients with unilateral Crowe-IV developmental dysplasia of the hip, correcting spinal and pelvic tilt, and improving patient gait.

Introduction: Spinopelvic kinematics, reflected by the change in spinopelvic tilt (ΔSPT) from a standing position to a flexed seated position, has been associated with the risk of prosthetic impingement and hip dislocation. Some studies have suggested changes in spinopelvic mobility after total hip arthroplasty (THA), but none have explored changes in mobility in the first three months following THA using a direct anterior approach.
