A girl in her middle childhood presented to the outpatient department (OPD) with a congenital melanocytic naevi (CMN) of the right nasal alar lobule. Her parents had aesthetic concerns and expressed their desire to get the lesion removed. The full-thickness excision of CMN was performed with the reconstruction of the defect using the nasolabial and dorsal nasal advancement flap with conchal cartilage to shape the contour of the ala.

Oral melanocytic nevi (OMN) are rare benign tumors originating from melanocytes with an unclear pathogenesis. The current theory suggests that OMN originate from dormant dendritic melanocytes that become enclosed in the dermis during the embryonic migration of melanoblasts - the precursors of melanocytes - from the neural crest to the epidermis. OMN can be congenital or acquired, with acquired nevi being more common.

Congenital melanocytic nevus (CMN) syndrome is a rare, non-familial neural ectodermal dysplasia characterized by CMN combined with extracutaneous abnormalities, predominantly involving the central nervous system (CNS). The pathogenesis of CMN syndrome is thought to result from early post-zygotic somatic mutations. CNS melanosis frequently affects the anterior temporal lobes, brainstem, cerebellum, and cerebral cortex.
