A 53-year-old male presented following cardiac arrest, followed by cardiopulmonary resuscitation. He was found to have myocardial infarction, bihemispheric cerebral embolization and mitral valve endocarditis. Mitral valve replacement was performed and was detected on PCR. This case represents a valuable addition to the limited reports on gonococcal endocarditis.

Gonorrhea is a sexually transmitted infection. Manifestations include asymptomatic infection and disseminated, life-threatening disease. We present two cases of mitral valve gonococcal endocarditis and discuss epidemiologic trends in disseminated gonococcal infection, acknowledging antimicrobial resistance, genetic variability in gonococcal strains, and contextual factors related to the COVID-19 pandemic and STI control.

Article Synopsis
  • * This report presents a case of culturenegative infective endocarditis caused by Neisseria bacilliformis, which was identified using 16S rRNA analysis of the affected valve tissue.
  • * A literature review indicated that beta-lactam antibiotics are effective against N. bacilliformis, suggesting a relatively favorable outcome for patients, marking this as the first recorded instance of identifying N. bacilliformis in culture-negative endocarditis through 16S rRNA analysis.

SummaryWe describe a case of culture-negative infective endocarditis due to , where the application of metagenomics shotgun sequencing in blood played a pivotal role in elucidating the underlying aetiology, guiding targeted therapy and ultimately resulting in the patient's complete recovery. Beyond its immediate clinical impact, prompt treatment bears significant implications for public health. The utilisation of molecular testing emerges as a valuable strategy to enhance diagnostic accuracy, particularly in cases involving fastidious organisms that are infrequently associated with infective endocarditis.

Introduction And Importance: Disseminated gonococcal infection (DGI) is an infrequent but serious complication of gonorrhea that can exhibit atypical symptoms. While rare, it can lead to infective endocarditis (IE), a condition that affects the heart valves and can result in severe and potentially life-threatening outcomes.

Case Presentation: We present a case of -caused IE confirmed by blood culture and direct isolation from the aortic valve vegetation.
