A 75-year-old man presented with cognitive decline, headaches, and ataxic gait. Magnetic resonance imaging (MRI) revealed acute infarcts in multiple brain regions, and vessel wall MRI (VW-MRI) demonstrated concentric arterial wall thickening and enhancement in some intracranial arteries, initially suggesting primary central nervous system vasculitis (PCNSV). Despite immunosuppressive therapy, the patient developed further infarction.

The presence of the hyperdense basilar artery (HDBA) sign, which indicates basilar artery occlusion (BAO), plays an important role in the early diagnosis and intervention in patients with acute ischemic stroke. However, qualitative and quantitative assessment of the HDBA sign is challenging. This case report describes a 60-year-old woman with a history of diabetes mellitus, hypertension, and cerebral infarction.

Embolic stroke due to pulmonary vein thrombosis is rare but may be associated with lung and left atrial tumours, pulmonary surgery, atrial fibrillation and radiofrequency ablation. It is very rarely idiopathic. A 23-year-old man developed acute onset of a left partial third nerve palsy and left ataxic hemiparesis.
