Background: Discussing advance care planning (ACP) preferences for nasogastric tube (NG) insertion remains challenging among older adults. Multimedia decision-making aids have demonstrated potential to enhance understanding and support informed decision-making.
Aim: To evaluate the effectiveness of the EATING program (multimedia decision-making aids + traditional educational module) compared to usual care (traditional educational module alone) in improving ACP readiness on NG insertion for community older adults with chronic illness.

Introduction: Cardiovascular disease (CVD) is a leading cause of mortality in the United States, disproportionately affecting marginalized populations such as Black and Latinx sexual minority men with HIV. These individuals face heightened CVD risk due to chronic inflammation related to HIV, side effects from treatment, and intersecting social disadvantages, including stigma and discrimination. Behavioral interventions specifically targeting these populations have been limited, with insufficient uptake in marginalized communities.

Before national guidelines related to infant feeding among people living with HIV (PLHIV) in the United States were changed in 2023, PLHIV were discouraged from breastfeeding due to the potential risk of perinatal HIV transmission. Potential health benefits to both mother and child coupled with the ability to mitigate HIV transmission risk with modern treatments now make feeding decisions more complex for PLHIV. This study examined personal and social determinants of feeding choice among PLHIV in the Philadelphia area just prior to the change in national guidelines.

Introduction: Postoperative delirium occurs in up to 40% of older surgical patients and has been associated with prolonged hospital stays, long-term cognitive impairment and increased 1-year postoperative mortality. Postoperative sleep disturbances may increase the risk of delirium, but studies investigating pharmacotherapies to improve postoperative sleep to prevent delirium remain limited. Suvorexant is a selective antagonist of orexin 1 and 2 receptors and is approved for insomnia pharmacotherapy by the Food and Drug Administration.

Purpose Of Review: Precision medicine is based on the idea that treatment can be individualized for each patient in a given clinical environment. This review summarizes factors that should be considered when clinicians are creating individualized plans for intubation, specifically focusing on physiologically difficult airways. Recent literature identifying physiologic risk factors is summarized, and individual and system-level interventions that can potentially mitigate risk are reviewed.
