Long-Term Outcomes of EUS-Guided Gastroenterostomy: A Large, Single-Center Experience.

Clin Transl Gastroenterol

December 2024

Division of Gastroenterology and Hepatology, University of North Carolina, Chapel Hill, North Carolina, USA.

Introduction: Endoscopic ultrasound-guided gastroenterostomy (EUS-GE) is effective and safe in benign and malignant indications. However, there is a paucity of literature on the outcomes of these patients. Our study evaluates the long-term outcomes of patients who underwent EUS-GE and stent-related adverse events (AEs).

The Role of Non-Coding RNAs in Regulating Cachexia Muscle Atrophy.

Cells

September 2024

School of Chinese Medicine, Li Ka Shing Faculty of Medicine, The University of Hong Kong, Hong Kong SAR, China.

Article Synopsis
  • Cachexia is a serious condition seen in various diseases that results in severe muscle and potentially fat loss, which can lead to higher death rates.
  • It is driven by several signaling pathways and molecules that increase muscle breakdown while decreasing muscle building and repair.
  • Non-coding RNAs, like microRNAs, long non-coding RNAs, and circular RNAs, are crucial in understanding muscle loss in cachexia and have the potential to serve as biomarkers and targets for new treatments.

Objective: Following hypoxic-ischemic (HI) brain injury, neuronal cytoplasmic chloride concentration ([Cl]) increases, potentially contributing to depolarizing γ-aminobutyric acid (GABA) responses, onset of seizures, and the failure of antiepileptic drugs that target inhibitory chloride-permeable GABA receptors. Post-HI seizures characteristically begin hours after injury, by which time substantial accumulation of [Cl] may have already occurred. In immature neurons, a major pathway for Cl influx is the reversible Na-K-2Cl cotransporter NKCC1.

Indications for Intestinal Transplantation.

Gastroenterol Clin North Am

June 2024

Intestinal Rehabilitation & Transplant Program, Icahn School of Medicine at Mount Sinai and Mount Sinai Hospital, One Gustave Levy Place, Box 1104, New York 10029, USA. Electronic address:

Outcomes for patients with chronic intestinal failure have improved with organization of experts into multidisciplinary teams delivering care in intestinal rehabilitation programs. There have been improvements in understanding of intestinal failure complications as well as development of newer therapies that have amplified the improvements in survival. In spite of this encouraging trend, patients who fail PN are often referred too late for intestinal transplantation.

Objective: Type II endoleak (T2EL) is the most common type of endoleak after endovascular aneurysm repair (EVAR) and a common indication for reintervention due to late sac enlargement. Although pre-emptive embolization of the inferior mesenteric artery (IMA) has been proposed to prevent this, no studies have prospectively demonstrated its efficacy. This study aimed to prove the validity of IMA embolization during EVAR in selective cases by analyzing the mid-term outcomes of a randomized clinical trial (RCT).
