Neurofeedback and epilepsy: Renaissance of an old self-regulation method?

Neurofeedback is a brain-computer interface tool enabling the user to self-regulate their neuronal activity, and ultimately, induce long-term brain plasticity, making it an interesting instrument to cure brain disorders. Although this method has been used successfully in the past as an adjunctive therapy in drug-resistant epilepsy, this approach remains under-explored and deserves more rigorous scientific inquiry. In this review, we present early neurofeedback protocols employed in epilepsy and provide a critical overview of the main clinical studies. We also describe the potential neurophysiological mechanisms through which neurofeedback may produce its therapeutic effects. Finally, we discuss how to innovate and standardize future neurofeedback clinical trials in epilepsy based on evidence from recent research studies.

Evaluation of mechanisms of action of EEG neurofeedback (EEG-nf) using simultaneous fMRI is highly desirable to ensure its effective application for clinical rehabilitation and therapy. Counterbalancing training runs with active neurofeedback and sham (neuro)feedback for each participant is a promising approach to demonstrate specificity of training effects to the active neurofeedback. Wearable EEG technology opens new avenues for consumer applications, such as mental health monitoring, neurofeedback training, and brain-computer interfaces. The objective of this study is to evaluate the impact of EEG biofeedback training under normoxic and normobaric hypoxic conditions on both simple and complex reaction times in judo athletes, and to identify the optimal training frequency and environmental conditions that substantially enhance reaction times in the examined athlete groups. The study comprised 20 male judo athlete members of the Polish national judo team in the middleweight and heavyweight categories.
