Sub-MIC levels of bedaquiline and clofazimine can select mutants with increased MIC. | LitMetric

Sub-MIC levels of bedaquiline and clofazimine can select mutants with increased MIC.

Antimicrob Agents Chemother

Published: April 2024

AI Article Synopsis

  • Multidrug-resistant tuberculosis (MDR-TB) patients who don't fully recover after treatment remain at risk of exposure to Minimum Inhibitory Concentration (MIC) and sub-MIC levels long after stopping drugs like bedaquiline (BDQ) or clofazimine (CFZ).
  • Experiments show that cultures treated with sub-MIC levels of BDQ and CFZ led to an increase in mutant bacteria, indicating that these lower drug levels can select for pre-existing drug-resistant mutants.
  • If these patients are not given another effective MDR-TB treatment, the potential for drug-resistant strains to thrive increases significantly.

Article Abstract

Multidrug-resistant tuberculosis (MDR-TB) patients not cured at the time of stopping treatment are exposed to Minimum Inhibitory Concentration (MIC) and sub-MIC levels for many months after discontinuing bedaquiline (BDQ) or clofazimine (CFZ) treatment. cultures treated with BDQ and CFZ sub-MIC concentrations clearly showed enrichment in the mutant population, demonstrating that pre-existing mutants can be selected by sub-MIC concentrations of BDQ and CFZ if not protected by an alternative MDR-TB treatment.
