Primary epithelioid trophoblastic tumor (ETT) of the lung is exceedingly rare. Only about three cases have been reported in Pubmed and worldwide literature. We presented a case of multiple primary ETT of the lung occurring in a 33-year-old Chinese female patient. Comprehensive physical examinations revealed no evidence of a primary lesion on the uterus or cervix uteri. Microscopic examination of the tumor demonstrated ETT of the lung, which was confirmed by immunohistochemical staining and declining level of beta-human choriogonadotropin ( β -HCG) after the operation. Our case revealed that the ETT can occur in the lung and should be considered when a female had a tumor of lung with increasing β -HCG.

Comparison of Inspiration-Synchronized and Continuous Vibrating Mesh Nebulizer During Adult Invasive Mechanical Ventilation.

J Aerosol Med Pulm Drug Deliv

December 2024

Department of Cardiopulmonary Sciences, Division of Respiratory Care, Rush University, Chicago, Illinois, USA.

Aerosol delivery may be enhanced by utilizing an inspiration-synchronized nebulization mode, where nebulization occurs only during inspiration. This study aimed to compare aerosol delivery of albuterol via a prototype of an inspiration-synchronized vibrating mesh nebulizer (VMN) versus continuous VMN during invasive mechanical ventilation. A critical care ventilator equipped with a heated-wire circuit to deliver adult parameters was attached to an endotracheal tube (ETT), a collection filter, and a test lung.

Background: An inspiration-synchronized vibrating mesh nebulizer (VMN) has been reported to improve aerosol delivery during adult mechanical ventilation. A prototype VMN generating smaller particles was developed. We aimed to compare the aerosol delivery efficiency of small-particle and conventional VMNs in inspiration-synchronized and continuous modes during neonatal and pediatric mechanical ventilation.

Objectives: The present study compared respiratory parameters between the two methods of airway establishment, ETT and LMA, for patients scheduled for orthopedic surgery with general anesthesia.

Methods: This randomized double-blinded clinical trial was conducted on patients scheduled for elective orthopedic surgery under general anesthesia, in Bandar Abbas, Iran, from January 2021 to December 2021. Using a random allocation table, the study participants were randomly divided into two groups, to employ either ETT (n=48) or LMA insertion (n=48).

Article Synopsis
  • Mucus plugging in respiratory diseases like asthma and cystic fibrosis can block airways, causing serious breathing issues.
  • A study tested a combination treatment called BromAc on mucus plugs in a lung model using sheep to see its effectiveness.
  • Results showed that BromAc significantly reduced airway resistance and viscosity of mucus compared to saline, leading to better airflow and suggesting it worked better than individual treatments.

Article Synopsis
  • The study investigates the characteristics and treatment outcomes of patients with gestational trophoblastic neoplasia (GTN) who initially present with lung lesions and high hCG levels but without visible pelvic lesions.
  • The analysis included 40 patients from a larger group of 2358, revealing that most had solitary lung tumors, primarily epithelioid trophoblastic tumors (ETT), and those treated with only chemotherapy faced higher recurrence rates.
  • It concludes that surgical removal of lung lesions is critical, and if chemotherapy is used, it should involve a multidrug approach to reduce the risk of relapse.
