Primary cerebral melanoma: An exceptional localization in a case report.

Radiol Case Rep

Radiotherapy Department, Mohammed VI University Hospital, Faculty of Medicine and Pharmacy, Mohammed 1st University Oujda, Morocco.

Published: May 2024

Cerebral melanoma is often related to a secondary location of a cutaneous or mucosal melanoma. However, primary cerebral melanoma is a very rare clinical situation, representing less than 1% of all melanomas and 0.07% of all cerebral tumors. The diagnosis of a primary cerebral melanoma therefore requires rigorous clinical and paraclinical investigations. We report a case of primary cerebral melanoma treated in our onco-radiotherapy department.

Background: Cutaneous melanoma is the leading cause of death from cutaneous malignancy and tends to metastasize lymphatically and hematogenously to the lung, liver, brain, and bone; it is a rare source of metastatic disease to the eye. Herein we provide a case report of cutaneous melanoma metastatic to the ciliary body and choroid involving clinical examination, slit lamp photography, and B-scan ultrasonography.

Result: A 55-year-old female with known metastatic cutaneous melanoma presented with pain, a large ciliochoroidal mass, visual decline, and diffuse intraocular inflammation.

Purpose: Melanoma brain metastases (MBMs) are a common, lethal complication of metastatic melanoma. Despite improvements in treatments, subsets of MBM patients experience rapid decline, and few prognostic biomarkers have been identified. An improved understanding of the molecular features specifically associated with MBM overall survival (OS) and intracranial progression free survival (PFS) could facilitate the development of more effective clinical management strategies.

Introduction: BRAF mutations are the most common driver mutation in cutaneous melanoma, present in 40% of cases. Rationally-designed BRAF targeted therapy (TT) has been developed in response to this, and alongside immune checkpoint inhibitors (ICI), forms the backbone of systemic therapy options for BRAF-mutant melanoma. Various therapeutic approaches have been studied in the neoadjuvant, adjuvant and advanced settings, and there is a wealth of information to guide clinicians managing these patients.
