We report the case of a 12-year-old girl with stage 4 neuroblastoma who developed tonic pupils secondary to immunotherapy with dinutuximab, an anti-GD2 antibody, based on the timeline provided by her mother with regard to onset of symptoms. The patient presented with difficulty reading and chronic dilated pupils bilaterally, according to her mother's observations over 6 months prior to presentation. Testing with dilute pilocarpine supported our presumption of tonic pupils.

Background: Transcutaneous vagus nerve stimulation (tVNS) has emerged as a promising technique to modulate autonomic functions, and pupil dilation has been recognized as a promising biomarker for tVNS-induced monoaminergic release. Nevertheless, studies on the effectiveness of various tVNS protocols have produced heterogeneous results on pupil dilation to date.
Methods: Here, we synthesize the existing evidence and compare conventional ("continuous") and pulsed stimulation protocols using a Bayesian meta-analysis.

A growing body of evidence across psychology suggests that (cognitive) effort exertion increases in proximity to a goal state. For instance, previous work has shown that participants respond more quickly, but not less accurately, when they near a goal-as indicated by a filling progress bar. Yet it remains unclear when over the course of a cognitively demanding task do people monitor progress information: Do they continuously monitor their goal progress over the course of a task, or attend more frequently to it as they near their goal? To answer this question, we used eye-tracking to examine trial-by-trial changes in progress monitoring as participants completed blocks of an attentionally demanding oddball task.

Alteration of responses to salient stimuli occurs in a wide range of brain disorders and may be rooted in pathophysiological brain state dynamics. Specifically, tonic and phasic modes of activity in the reticular activating system (RAS) influence, and are influenced by, salient stimuli, respectively.
